Role of the Services Coordinators Regarding Nutrition Services • Determine Waiver eligibility • Assess/identify nutritional needs • Discuss needs and explain benefits to client/family • Authorize services • Develop Nutrition Plan with client • Contact Resource Developerfor names of nutritionistsin community • Determine who will contact Nutritionist
Authorization of Services by Services Coordinator • First visit: Assessment & Development of Nutrition Plan • Up to 5 additional visits within 3 month period as indicated in Plan • At end of authorization period can authorize 3 additional months if needed
Role of Licensed Medical Nutrition Therapist (LMNT) or Certified Diabetes Educator (CDE) • Assess Client’s Nutritional Status • Develop Nutrition Plan with Client • Counsels, Intervenes, Follow Up • Provides Documentation to Client and Services Coordinator
Nutritionist Client Services Coordinator Other team members (family members or friends) Client’s Physician Who Is Involved in the Development of Nutrition Plan?

Who Receives Copy of Nutrition Service Plan • Client’s physician if physician-identified needs • Client • Services Coordinator • At beginning when the Nutrition Plan is initiated • On-going communication • At end of three month authorization period
Services Coordinator receives progress outcomes summary after initial 3 months Services Coordinator may authorize an additional 5 visits over next 3 months Monitoring the Nutrition Plan
Services must be terminated when: Outcomes of Nutrition Plan have been met Client is clearly notbenefiting fromNutrition Services Client requeststermination of the service Termination of Nutrition Services
Current Nutrition Service Rates are set by Central Office Be sure to check withsupervisor for currentrate amount Rates may change annually Nutrition Services Rates
The Nutrition Service Provider mustmaintain a file that contains: Nutrition Plan and allrecommended changes Progress reports Name of the client’s physician All pertinent medical information Record Keeping
Maintains an active list of Nutrition Service Providers Provides a current list of the Nutrition Service Providers to client Recruits new Nutrition Service Providers Role of Resource Development
