  7. Frail elderly - dementia • Dementia Care • Early diagnosis • Memory assessment service • Dementia support service • Dementia care at RDH • Care home support

  8. RightCare • Significant increase in number of plans • DHU believe excessive number • Many patients could be adequately managed with Special Patient Note • ?Need to review use of RightCare
