Purpose of the Program Agencies receiving federal funds are mandated to meet all requirements of Section 504 of the American with Disabilities Act. The purpose of the Section 504 Program is to ensure that no student identified with a handicapping condition is excluded from participation in, denied the benefits of or is subjected to discrimination under any program or activity in the Park Hill School District based solely on their handicapping condition.
Student and/or stakeholder needs addressed by the program • Students identified with disabilities are afforded a 504 Learning Plan which outlines the accommodations necessary to ensure equal access to all programs and activities in the educational setting. • Staff is required to provide each accommodation listed in the student’s 504 Learning Plan across all school environments.
Service Delivery Model Students with disabilities are referred to the 504 Coordinator in their building. An evaluation is completed to determine if the student’s disability has a significant impact on a major life function (learning). The 504 team meets and develops a 504 Learning Plan for all students determined eligible. Students, parents and staff meet throughout the school year to determine the effectiveness of the plan Adjustments are made as needed.
Progress Toward Previous Program Goal GOAL: The 504 Program performance objective for 08-09 was to increase the consistent implementation of accommodations listed on the student’s 504 Learning Plan from 75% of students with learning plans to 90%. RESULTS: Data indicates that 90% of all 504 Learning Plans written during the 08-09 school year were consistently implemented.

GOAL STATUS: The program goal was met DEPLOYMENT LEVEL: Services are well deployed, although deployment may vary in some areas or schools.
Overall Goals of the Program To meet all regulatory standards mandated in Section 504 of the American with Disabilities Act.
