    3. Risk Prevention and Response, HFHI 2007 The mandate of the Quality Assurance program is to ensure uniformity and consistency among HFHI affiliated organizations throughout the United States. This program consists of two major components, the QA Checklist and the US Policy Handbook. Affiliates are expected to use the policies referenced in the US Policy Handbook to bring their own affiliate practices and policies into compliance with what is expected of all US affiliates.

    4. Risk Prevention and Response, HFHI 2007 The Quality Assurance Checklist is just one of four important Quality Assurance building blocks. The others are: U.S. Affiliation Agreement U.S. Affiliated Organization Covenant U.S. Affiliated Organization Policy Handbook Electronic copies of the most current versions of these important documents are available in the US Office section on PartnerNet Beta.

    5. Risk Prevention and Response, HFHI 2007 History of the QA Program 2005: HFHI’s board of directors charge senior leadership to manage risk and to promote good governance practices. The Risk Prevention and Response Unit is formed which is led by Starr Mayer. U.S. Office forms policy committee of staff and affiliate representatives to assemble the Policy documents that will govern affiliate work.

Risk Prevention and Response, HFHI 2007 History of the QA Program (cont’d) 2006: After feedback from staff and affiliates, the 1st version of Policy Handbook was adopted by the US Council (Sept) 2007: Hard copy US Policy Handbook sent to all affiliates (Jan) QA Checklist received final edits in May. New policies adopted in Sept Handbook updated for consistency with US AA

    7. Risk Prevention and Response, HFHI 2007 What is the Quality Assurance Checklist? A self-assessment tool Based on the six Core Tenets Represents the minimum performance standards for U.S. affiliates. Designed to complement the policies in the US Policy Handbook. Replaces the old Standards of Excellence and the Triennial Review.

    8. Risk Prevention and Response, HFHI 2007 Who should complete the QA Checklist? A broad-based group of representatives from the affiliate will be most effective It can be an educational tool that can help affiliates review procedures and policies that are currently in place and identify areas needing improvement. It is an opportunity for dialogue and discussion among key stakeholders, especially in areas that need improvement.

    9. Risk Prevention and Response, HFHI 2007 QA Checklist response “Yes” or “NO” The Checklist consists of standards to which affiliates are to answer either YES or NO Yes indicates that they are adhering to the standard No means they are not adhering to the standard.

    10. Risk Prevention and Response, HFHI 2007 What if the answer to a question is “yes" and “no”? There will be multiple choices for affiliates to select if responding “no.” By selecting the most accurate response, the U.S. Office will best be able to support an affiliate’s training needs. Data gathering is a key component of the QA Process

Risk Prevention and Response, HFHI 2007 “No” Explanations NO – Our affiliate does not have this in place but we would like assistance in this area NO – Our affiliate does not have this in place but policy/procedure change in this area is in progress/pending board resolution NO -- Our affiliate does not have this in place and we understand this means we are not adhering to the standard, but there are no plans to change

    12. Risk Prevention and Response, HFHI 2007 What if an affiliate does not meet the minimum standards? In FY ‘08 (July 1, 2007, to June 30, 2008), affiliates are encouraged to build systems necessary to be able to check “yes”. Affiliates are asked to complete the QA Checklist completely and return it to HFHI (Jan 31st). Links to supporting docs and resources will be available on partnernet Beta If an affiliate checks “no” on any response, information and assistance are available through HFHI (see the links). Checking “No” will not be penalized in FY 08! QA Team is being formed and its members will be following up with affiliates on “No” responses.

    13. Risk Prevention and Response, HFHI 2007 When will affiliates HAVE to have all “Yes” answers? By FY09 the expectation is that all US Based affiliates are meeting minimum standards QA Team will be looking at data to determine if changes to standards should be made QA Team will provide senior leadership with data

    14. QA Checklist Section I Covenant Habitat affiliates are formed in a community as a Christian ministry that not only builds homes for those in need but also transforms communities.

    15. QA Checklist-Section 2 Service Delivery Habitat affiliate programs support and actively work to accomplish its mission.

    16. QA Checklist-Section 3 Governance Habitat affiliates are governed by a board of locally elected volunteers who are committed to the affiliate’s mission, determine policies and procedures, and monitor the affiliate’s financial and programmatic performance.

    17. QA Checklist-Section 4 Human Resources Habitat affiliates ensure that there are adequate human resources to accomplish the mission.

    18. QA Checklist-Section 5 Financial Management and Legal Habitat affiliates practice sound financial management and comply with various legal and regulatory requirements.

    19. QA Checklist-Section 6 Development Fund-raising activities adhere to professional and ethical fund-raising practices.

    20. QA Checklist-Section 7 Partnering Habitat affiliates understand and articulate the larger context of housing needs and actively partner with others to eliminate poverty housing.
