MALAYSIA’S SUCCESS IN BRINGING DOWN THE MATERNAL MORTALITY RATE MULTIPLE FACTORS : • POLITICAL STABILITY AND POLITICAL WILL • FREE EDUCATION FOR WOMEN • SOCIO-ECONOMIC DEVELOPMENT • REDUCTION IN FERTILITY SINCE FAMILY PLANNING PROGRAMME STARTED IN 1971 • IMPROVED MATERNITY SERVICES (BETTER QUALITY, OUTREACH STRATEGIES, IMPROVED CULTURAL ACCEPTANCE)
Safe Motherhood Initiative implemented in 1989 a. Risk Approach i) Training and utilization of traditional birth attendant – slowly phased out now ii) Orientation of health/hospital personnel iii) Organized referral, feedback and follow-up systems at PHC level–strengthened
… Continuation iv) Introduction of colour coding to indicate risks in pregnancy and standardization of management procedures for obstetric emergencies. v) Empowerment of health personnel to make decisions and refer high risk ante-natal mothers to higher levels of care.
b. National Committee was formed to monitor the Maternal Health Services and to plan programs. c. State and District levels – committees formed to set targets to reduce morbidity and mortality and to plan strategies. d. Confidential enquiry into maternal deaths – anonymity encouraged accurate reporting of cases and learning opportunities.
e. PILLARS OF SAFE MOTHERHOOD - Ante-natal care, safe delivery, post-natal care, family planning f. 1995 - Health advisory panel set up for community participation. g. Introduction of maternal home-based card for individual health monitoring and improved quality of documentation . h Safe delivery by trained midwives and assisted by traditional birth attendants - cultural observances and wholistic care.
WHAT IS THE ROLE OF MALAYSIAN NURSES ASSOC. (MNA) ? • To form Special Interest Group for Midwifery (at present there is Paediatrics and Public Health) • Organize seminars and workshops which are related and forward recommendations from the grass-roots. • Actively involved with continuing professional development for its members. Scolarships awarded to members by MNA’s arm MNF
CONCLUSION 1. Far-sightedness of government. Good infrastructure 2. Use of traditional birth attendants as an ally for cultural observances. 3. Identifying and training of key personnel 4. Knowledge and skills of staff enlarged and enhanced 5. Implementation of APC and enforcement of CPD points to improve nurses knowledge. 6. Last but not least credit must be given to the nurses and midwives in public health who provide the basic health care and are the front liners in the primary health care service.
