3. Integrated Disability Service Launched April 2006
Staff co-located January 2008 under single line managed service with one front door
IDS has brought together services working with disabled children, young people and their families from education, social care, connexions and health.
Teachers, Teaching assistants, Social workers, Occupational Therapists, Physiotherapists, Connexions specialist advisers, Child Development Teams, Family key workers, Short Break team (Aiming High).
4. Integrated Disability Service Senior leadership team - made up of managers from a range of backgrounds: - Social Care, Voluntary Sector, Connexions, Teaching, Health.
This integrated working gave a platform which made multi agency working within transition easier to achieve.
5. Multi Agency Transition Responsibility for transition shared by 2 senior managers one in IDS and one in adult services.
Transition processes encompass broad range of young people who need support during this time not just those meeting social care criteria.
Information/data collected feeds into planning and commissioning.

6. Multi Agency Transition - Structure Multi Agency Transition Group
Purpose: To provide a strategic overview of transition work across the county.
Joint Chairs: Simon Veasey Adult Services / Diana Sellwood IDS
Attendees: Managers from key agencies involved in the transition process. Parent/carers are represented and young peoples views are sought via a link with the Wacky Forum
Meets: Bi monthly
Reports to: The Learning Disability Partnership Board, Children and Young People Strategic Partnership Board, the IDS Parents Steering Group and the Wacky Forum
7. Multi Agency Transition - Structure Locality Transition Groups
Rugby , North Warwickshire, Central Warwickshire, South Warwickshire
Purpose: Networking, sharing information and joint training.
Attendees: Practitioners from across the relevant agencies and parent/carers
Meets: Once a term in local special schools
8. Multi Agency Transition - Structure Year 8 Transition Meetings
Chairs Members of MATG
Purpose: To consider the needs of all young people with a statement of educational needs and agree a lead agency to support each young person through transition from school.
Attendees: School SENCOs, Managers from IDS teams, Connexions managers.
Meets: Meetings held annually in mainstream and special schools across the county during June/July
9. Multi Agency Transition - Structure Transition Case Planning Meetings
North and Rugby
Centre/South
Purpose: To plan the transfer of individual cases from IDS to adult services
Attendees: Team Leaders from IDS Social Care, IDS Connexions, Leaving Care, Adult Services
Meets: Meetings held every 6 months.
10. Multi Agency Transition - Structure Transition Information Events
Purpose: Provide parents/carers with information about transition and services available to young people moving on into Adulthood.
Attendees: Parents/carers of young people in Year 8 and above with a disability
Where/When: Every two years in each locality
11. Multi Agency Transition - Structure Transition Training Workshops
Purpose: Provide practitioners with training around Transition and the opportunity to network
Attendees: Multi-agency services that work with young disabled people in Transition
Where/When: Annually South and North

13. Autistic Spectrum Condition Transition Pathway Plan Data gathered from year 8 meetings and case planning meetings used to inform commissioners within adult services of gap in services for adults with high functioning autism/aspergers.
Led to development of:

Autism broker service
14. Autistic Spectrum Condition Transition Pathway Plan Adult Learning Disability take lead in assessment under Fair Access to Care.
If meet criteria of high or substantial adult LD provide service
If do not meet criteria then will be referred to Autism Broker service
Autism Strategy - likely to lead to further developments in this service area.
