The Diabetes Care and Outcomes Audit, 2015 • Patients to include in the Audit: • Have diabetes • Are Alaska Native/American Indian • Had a visit for primary care in 2014 • Patients to exclude: • Received most of their care elsewhere • Are on dialysis and receive their care at that clinic • Have died before the end of the audit period • Have gestational or pre-diabetes • Have moved, temporarily or permanently Northwest Portland Area Indian Health Board

SDPI Tribal Competitive Successes • Every group had substantial weight loss • With that lower triglycerides, blood pressure, cholesterol • Emphasis on the role of the coach and support • Education was essential • Participants have carried message to family • In event not eligible for program – clinic support • Participants were able to incorporate into life • 1st group have become mentors and coaches to 2nd group • Four tribes in Oregon (Cow Creek, Klamath, Coquille – consortium and Warm Springs)
