Providing feedback • We asked you what you thought of the hospital • We worked out what changes to make and discussed that with you • Now we are letting you know what happened when after we made the changes

One Example (Story) • One hospital was not sure where Aboriginal patients wanted to be discharged to for ongoing treatment • AHLO rang patients who had been to the emergency department in the last 3 months • Asked them where they were discharged to and if it worked • Asked them where they would have preferred to be discharged to • Created list of agencies that Aboriginal patients preferred for nurses to use in ED when discharging patients • Check in 6 months if this is still OK
