Community Engagement & OutreachCommunity engagement is the ingredient for harmonizing efforts of members of the lay community, community non-profit organizations, health practitioners and medical and public health professionals to improve health. True community engagement is a repetitive process where all parties negotiate continuously.
Community Outreach SpecialistGoal: To establish community partnerships with Health System Leadership (Physicians, Healthcare Administrators, Medical Directors), Patients, and Clinical Staff to implement and sustain improvements in health care.

Community Outreach Specialist Goal for the Year: 1. Engage in collaboration & establish equitable partnerships (i.e., evaluate a program offering questionnaires to determine eligibility for diabetes screening to patients receiving annual flu shots). 2. Build on the resources and goals already present (i.e., improve patient care coordination, implement an electronic health record, improve eye screenings). 3. Create an Invest in Sustainable and Robust partnerships….i.e. culturally appropriate diabetes messages in all languages.
