Problem Statements & Potential ‘Solutions” • 7 million South Africans infected with HIV • An estimated 270 000 new infections per year (740 per day) • 3.9m on treatment • Current annual spend on HIV is R32b (still 8% under budget!) • How to get the best value for this investment? • Investment cases for HIV and TB • Focus for impact

Investment Cases for HIV & TB • What interventions should we prioritise to decrease new infections and decrease mortality & to reach the 90-90-90 targets as well as the longer term targets? • For HIV: • Condoms, VMMC, treatment, SBCC • Must ensure adherence to treatment • Will achieve 90-90-90 targets by 2020 but not WHO targets for 2030 (need new interventions)

Investment Cases for HIV & TB, contd • For TB: • Do the basics at scale and quality, focus on closing the gaps in the TB cascade • Key is finding the estimated 150 000 people with undiagnosed TB • Funding over the next 5 years needs to increase by 48%, thereafter required levels of funding declines as TB burden gets under control • Will only reach the 90-90-90 targets if we deal with both the HIV and TB epidemics simultaneously

Focus for Impact • Go where the virus/bacteria is – focus for impact! • Identified 27 high burden districts (HIV) and 20 high burden districts (TB) – with significant overlap – not surprising given the 60% co-infection rates • Focus at facility level – use granular level data to target interventions

  6. Conclusions • Back to basics • Know your epidemics • Search for key interventions to curb the epidemics (cannot do everything, everywhere) • Targeted interventions using granular level data
