  1. Program Evaluation Plan Report Board of Education Meeting May 10, 2012 Employee wellness

  2. Purpose of the Program • This program offers an array of screenings, educational programs and health services to employees. The main focus is to support employee satisfaction, enhance workforce culture, increase self-awareness and provide guidance and resources for individuals to improve health and wellbeing. Employees that are satisfied and of good health are more productive and better able to positively engage students.

  3. Service Delivery Model • Program information is delivered via building Health Champions, the Park Hill intranet, bulletin boards in each building, e-mails to all staff, promotion within the fitness center and on an individual basis.

  4. Future Program Goals to be Evaluated • Achieve high employee satisfaction. • Maintain overall employee satisfaction with the wellness program via an annual Health Culture Survey. The 2012 survey indicated overall satisfaction at 98%. • Maintain participant satisfaction in all programs at 98%. The average satisfaction among all programs since July 1, 2011 is 98%.

Future Program Goals to be Evaluated • Maintain high-level participation in the health screenings and health risk assessment. • Maintain participation in the health screenings and health risk assessment at 96%. • Participation over the previous three years is as follows: 34% in 2008, 92% in 2009, 96% in 2010 and 96% in 2011.

Future Program Goals to be Evaluated • Continue to offer programs and resources to employees based on the top three health risks as determined by annual health screenings and the Health Risk Assessment. • Offer at least two programs per health risk to provide guidance and resources to promote behavior change in that area. • Offer at least one multi-session/ long-term program per health risk to provide guidance and resources to promote behavior change in that area. Such long-term programs provide additional support and follow-up care to aid in positive behavior change.

Plan for Evaluation Measurable Outcomes Data Collection Methods • Maintain high employee satisfaction at 98%. • Maintain health screening and health risk assessment participation at 96%. • Surveys and questionnaires • Document reviews • Focus groups • Annual health screenings and health risk assessment
