Current Employee Wellness Options • H2NH does offer insurance plans through Aflac • Only 29% of respondents are insured by H2NH plan(s) offered • Hearts does not currently have an Employee Wellness Program

OBJECTIVES: • To present the FOREVER WELL Employee wellness program to Hearts to Nourish Hope which will: • Address the health and wellbeing of the employees • Be supported by Deborah Anglin, CEO • Be accepted and regularly utilized by the employees • Be practical, convenient, affordable, and sustainable

Overview of Methods Obtained and analyzed employee demographics and zip codes (separately to ensure confidentiality). Retrieved mortality and morbidity rates for the majority cohort groups at H2NH (data based on the State of Georgia obtained through OASIS). Conducted an anonymous and confidential employee survey via QualtricsSurvey Software online. Analyzed and assessed the overall health of H2NH employees per survey responses. Assessed employees level of interest and willingness to commit to various wellness program activities. Also per survey responses. Created significant and quality recommendations based on overall survey analysis and available resources within the community.

Mortality and Morbidity Rates • Data collected from OASIS based on State of Georgia data • Retrieved data on the following cohorts specific to H2NH employee base: • African American Males ages 19-29 (18%) • African American Males ages 50-59 (14%) • Caucasian Females ages 40-49 (18%)
