  1. FRA Financial Statement (Hard Copy Version) The Hard Copy version of the FRA Financial Statement is set up almost identical to the On-Line Submission program. It is 1 page (front and back) which includes the declaration Bank Statements will need to be submitted along with the financial statement

Hard Copy Version (front) Insert the calendar 2010 etc FRA Number and Tax ID Number Write in FRA Name Insert Net Assets from previous year Add up all receipts and place total here Add net assets from previous year (line #1) to total current receipts and place here Net assets from previous Year + Total current receipts Minus Total current expenditures = Net Assets Add up all current Expenditures

Hard Copy Version (rear) Write in your investments as they apply Check the box that applies to your FRA Governing Body and write the name of the Senior elected official Check the box that applies to the form of Governing Body and Fire Dept Name Write in your annuity program value and your pension program value Check the box that applies to your type FD # of Firefighters Fire Apparatus and Equipment value Write in Savings Account value Enter in any other investments Check the box to indicate that you have an attorney certification for any expenditures over $1,500 Check the box to indicate that you have a Treasurer Bond Add up all investments and total here Check this box for your declaration and verification
