SAFE RETURN • Nationwide identification, support, and registration program • Managed by the national Alzheimer’s Association • Funded, in part, by US Department of Justice grant • Operated 24 hours a day / 7 days a week • 1-800-572-1122
Triad Alzheimer’s Collaborative Plan of Action • Establish a mechanism for SAFE RETURN registrations throughout Cape. • Train volunteers to conduct registrations. • Establish a common referral line. • Launch a marketing campaign for Alzheimer’s awareness:Radio, Public Access TV, Newspaper • Establish County SALT Council subcommittee on SAFE RETURN to meet biannually.
Alzheimer’s Collaborative Roles Sheriff’s Office:Referral Line (508) 375-6043 Triad volunteers: Home visits for registration (some towns may have police or COA staff conduct home visits) Alzheimer’s Services: Process applications & Pay for registrations Alzheimer’s Association: Manage Registry, pay for marketing campaign & send letter to caregivers with additional resources.

Referral Process • Family calls BCSO referral line & leaves message. • Triad Director gets paged when message is left. • Triad Director gathers message info and calls P.O.C. in town where registrant lives. • Local P.O.C. calls family and makes arrangements for a registration visit.
Registration Visit • How did you hear about SAFE RETURN? • Review cover letter • Review info sheet & answer questions • Sign release form • Fill out registration form • Take photo (not mandatory) • Give resource phone numbers • Mail info to Alzheimer’s Services in Hyannis

TASKS • Each town needs to select a P.O.C. If P.O.C. is a volunteer, you need to recruit others to assist in home visits. Use the buddy system! • Each P.O.C. should take 10 registration packets with them. • P.O.C.s need to communicate with their police departments to determine if a local registry exists, and if there is a separate local registration form. • P.O.C.s should see if a camera is available from the police department or council on aging. If not, contact David Vieira and we can work something out.
