Personal Assistance Outreach Education Personal Assistance with eligibility & enrollment: • Health Insurance Marketplace • NE Medicaid (including expansion) • Lancaster County General Assistance • FQHCs 2018 CHIP Goal: Utilize patient advocates who personally assist people in accessing appropriate care & services

Personal Assistance Outreach Education Matt Talbot Kitchen & Outreach Prescott Elementary Yazda Community Center Wellcare of NE Community Baby Shower Center for People in Need Play in the Park: Parks & Rec Head Start/Early Head Start Malone Back to School Event LPS EduCare Ultimate Block Party in Calvert Salvation Army Resource Fair Arnold School’s Bike Rodeo Gathering Place Streets Alive in Belmont Cause Collective Project Homeless Connect Jazz in June Re-Entry Fair at York Correctional Center Juneteenth ESL Class at Randolph & Campbell Schools Cooper Park Party & Resource Fair The Bay Employment Fair Outreach at 60 community events in 2018!

Personal Assistance Outreach Education Education Increase the knowledge of health benefit resources in our community so that local case workers can connect their clients to a source of care rapidly. “Connect the Uninsured "Conference Cause Collective Training LPS Social Workers Staff Conference 196 local case workers attended a training in 2018!
