Medical Screening • Many clients are coming to receive a vaccine and need to be screened for contraindications to vaccination, therefore a whole station is allocated to Medical Screening. • This PowerPoint document reviews the role of the Medical Screener and the tools and resources that will help you to be a knowledgeable and effective Medical Screener. • These slides DO NOT replace Just in Time Training, but are a pre-clinic supplement.
Job Description: Medical Screener • Reports to: Medical Screener Supervisor • Mission: Screen Registration Forms and determine type of vaccine client should receive • Qualifications: Medically licensed; nursing preferred
Medical Screener Tasks • Review the medical part of every Registration Form • Using the algorithm, determine whether client should get Flu Mist, thimerosal free injection, or multi-dose vial injection • Mark the form for which type of vaccine the client should receive
Medical Screener Resources • Each Medical Screener will have a folder of information available to them at the clinic. • The folder includes a large amount of medical information and tools for the screening process. • The following slides review the Medical Screening tools and the last slide provides links to resources.
Medical Screener Tools • Each folder includes tools developed or adapted by the Franklin County Board of Health for specific use at the mass clinics • Tools include: • Medical Screener Procedure • Sample Registration form • Allergen Chart • Screening Algorithm
Medical Screener Procedure The Medical Screener Supervisor will review this procedure during your job-specific Just in Time Training. The Procedure on this slide is an example of a procedure document. IMPORTANT: This is THE PROCEDURE to be followed with EVERY client. It is designed so that all Screeners treat all clients equally and that every Screener is completing their role in the clinic team. Each screener will have a copy of this document so that it is always available. Follow all steps.
Sample Registration Form The Registration form is the document that the Screener uses to communicate with the Vaccinator. The client completes the front side of the document – bold areas MUST be completed for accurate screening. The Screener completes the back side of the document, communicating to the Vaccinator what vaccine is safe and appropriate for that client. The back of the registration form is designed to reduce errors, but the Screener must still be diligent in their screening and documentation for the Vaccinator to know what vaccine is safe and appropriate.
Allergen Chart The Nursing Department at the Franklin County Board of Health reviewed all of the ingredients of all brands of vaccine offered at the mass clinic. This chart reflects which brand of vaccine is safe if a client declares an allergy. If the allergen is not listed on the chart, then the allergen is not a component of any of the vaccines. After evaluating which brand is safe for the client with allergies, the Screener must still evaluate whether that type of vaccine is appropriate for the client (ie, if Flu Mist is the only brand that does not contain the allergen, does the patient’s medical history and age allow for them to have Flu Mist or not?)
Screening Algorithm This algorithm is designed to follow the screening questions the client answered on the front of their Registration Form. The Screener is expected to review ALL screening questions in order to evaluate “yes” answers and in order to give the client the opportunity to ask questions (not all client understand the language used in the screening questions). The algorithm does include extra information for the Screener to help evaluate the “yes” answers. The Screener should also refer to the Medical Screening Supervisor if uncertain of how the “yes” answer affects vaccine choice.
