Community Supports Program • 1. Program Mandate: • The Community Supports Program provides financial/ supportive services and case management for individuals with physical and/or intellectual disabilities, and individuals requiring protection under the Neglected Adults Act. • The program’s goals are to support individuals/ families/ caregivers and promote: Independence, community inclusion, safety, well being. • Based on the belief that individuals with disabilities have the right to full participation in the community and should be supported in the least restrictive environment.
Community Supports Program • 2. Referral Process: • Anyone can make a referral • - Self/professional/community referral • Medical verification of disability is required • Contacts for Referral: • Eastern Health Urban -Intake Coordinator: 752-4717 • Eastern Health Rural – Local Office

Program Eligibility: • Intellectual Disability: • Clinical diagnosis of intellectual disability • significantly sub-average intellectual functioning (IQ score more than 2 standard deviations below the mean for the IQ test administered) • related limitations in two or more of the following applicable adaptive skill domains: • functional academics; • communication; • self help/ daily living; • gross/ fine motor; • social/ emotional • manifesting itself before age 18 years • Assessed need for support with activities of daily living • Client must be financially eligible
Community Supports Program • 3. Program Eligibility: • Physical Disability: • Aged 18-64 yrs • Clinical diagnosis of acquired or congenital disability • Assessed need for support with activities of daily living • Client must be financially eligible.

Program Eligibility: • Neglected Adults • “adult” is over age of 16yrs • “neglected” defined as follows: • incapable of caring for self as result of physical or mental infirmity • not suitable for treatment under Mental Health Act • not in receipt of proper care/attention • refuses, delays or incapable to make provision for proper care • * must meet ALL four criteria
Community Supports Program • 4. Assessment • Assessment Process • All clients must have a clinical assessment and a financial assessment. • Services implemented when there is a demonstrated need for supportive services and when the client is financially eligible.

Supportive Services & Residential Options • Clients may reside in any of these residential options: • Own/Rented home • Board & lodging with relatives • Board & lodging with non-relatives • Individualized Living Arrangement - Intellectual Disabilities only • Alternate Family Care (AFC) – Intellectual Disabilities only • Cooperative Apartments - Intellectual Disabilities only
Community Supports Program • Supportive Services & Residential Options • Based on assessed need and financial eligibility, clients may qualify for • Home support (respite, homemaking, personal care) • Residential respite (weekends & vacation) • Personal care allowance • Other financial benefits • Drug card • Medical equipment and supplies • Intervention Services

Additional Social Work Roles & Responsibilities • Assistance with housing/placement breakdowns • Case management • Service Coordination • Interdisciplinary team approach • Resource counseling • Supportive counseling • Training (staff, AFC caregivers, etc.) • Family maintenance (encourage family involvement) • Internal & external referrals
