The Southwestern PA HCQU Serves • Allegheny • Cambria • Fayette • Greene • Somerset/Bedford • Washington • Westmoreland counties under the direction of APS healthcare.
Funding • Grant from Office of Mental Retardation (OMR) was awarded to the eight aforementioned counties • APS Healthcare was awarded the contract to fulfill the conditions of the grant by these counties
HCQU’s Vision Statement • PURSUIT OF QUALITY is the fundamental driving principle for Pennsylvania’s Developmental Disabilities Services System
HCQU’s Mission Statement • To serve as the entity responsible to county MR programs for assessing, supporting, and improving the overall health status of individuals receiving services from county programs
HCQU’s Goal • To support and improve the health care service system and it’s ability to meet the needs of individuals with mental retardation
HCQU’s Scope of Work • Assessing health needs of consumers • Analyzing data/developing trends • Training • Event tracking • Community capacity building
HCQU’s Data Collection • Eventually health risk assessment data will be collected on all individuals with mental retardation receiving any county services • In licensed residential facilities, approximately 2,611 individuals will be surveyed over a 3 year period • Assessment data will be used to determine needs for specialized training
HCQU’s Electronic Data Base • Data will be collected by HCQU field nurses-a full RN staff • Research analysis and trending will be performed • Statistical and analytical report writing will be performed by information system (is) for dissemination to the counties
HCQU’s Training • Design and implementation of training is a core function of HCQU • A full capacity of multimedia technology for education will be provided
Training Modules Current modules relate to: • Diabetes • Seizure disorder • Dysphagia • Psychotropic drugs • Smoking cessation • Anatomy of a behavior plan 7. Autism • Heart disease • Traumatic brain injury • Weight management • Prevention of falls
HCQU’s Event Tracking • Develop trends and patterns from incident report data analysis • Provide training that can impact outcome of incidents
HCQU’s Capacity Building/Access • Consultant medical director who is familiar with mental retardation, will assist with capacity building • Community resources that improve the health of consumers and assist with capacity building will be inventoried
Behavioral Health “Best Practices” • Best practices for the dually diagnosed population will be available for consultation through HCQU physicians’ expertise and collaboration with the state and regional dual diagnosis networking group
Activities Not Part of the HCQU • Provide direct or primary health care • License or regulate activities • Replace established community resources • Manage crises • Diagnose • Provide direct consultation
