STUDENT FUNDING FUNDED BY GSBS Funded for two year (8/18/2014--8/31/2016) FUNDED BY DEPARTMENTS, ADVISORS, OR PROGRAMS FROM UTHealth OR MDA Funded for 3 tutorials (8/18/2014--5/31/2015) After 3rd tutorial, you will identify a lab that will continue funding MS students are self funded, but may be funded by GSBS faculty from UTHealth or MDA

  4. TUITION AND FEES Funded Students: Tuition and Fees are paid by your funding source (Sponsorship Form) GSBS, Your Advisor, Program, Department Self Funded Students: Tuition and Fees are paid through myUTH

  5. PAYMENT OF TUITION AND FEES Funded Students: Sponsorship Forms are due from your Faculty/GSBS/Programs no later than 5 p.m. Friday, 8/22/2014 CHECK myUTH for “0” Balance Late Fees: $25 for Late Registration $25 for Late Sponsorship Form/Payment Students are Responsible for all Late Fees

INTERNATIONAL STUDENTS ALL International students and Permanent Residents must check-in with UT Office of International Affair (OIA) If you have not checked in with OIA YOU MUST DO SO THIS AFTERNOON UTHealth funded: Cleared by OIA, I-9 Finalized in HR MDA funded: Cleared by OIA, receive OCOS letter, take to MDA VISA office, and sign I-9. Tutorials at MD Anderson: Receive paperwork from OIA (OCOS)to present to MDA Visa Office.

International Students who need a SOCIAL SECURITY CARD Letter from OIA and Letter from funding institution to Social Security Office UTHealth….Elisabet Lau MDA……Carol Helton Receipt from Social Security Office Show receipt to Carol Helton (AVA/GSBS) at MDA OR Human Resources at UTHealth When you have original SS card, take UTHealth HR & Sherry Moser in Payroll & MDA to AVA/GSBS

  8. MDA Funded Students Your funding starts today 8/18/2014 PAY CHECK DATES Pays on 5th and 20th of each month UTHealth Funded StudentsYour funding starts today 8/18/2014 PAY CHECK DATES Pays on 1st and 16th of each month

  9. HEALTH INSURANCE ALL STUDENTS MUST HAVE HEALTH INSURANCE EFFECTIVE 9/1/14 FUNDED STUDENT (Employee Insurance) MDA–EFFECTIVE 9/1/14 UTHealth –EFFECTIVE 10/1/14

STUDENT INSURANCE Auxiliary Enterprises (713)500-8400 Application for ONE Month $155 (UTHealth Only) Health Insurance Certification every Semester myUTH BILL: Student Health Insurance-$708 for Fall Semester INTERNATIONAL STUDENTS are required to pay Medical Evacuation/Repatriation --$40 EVERY semester
