This report provides an overview of the ETA's safety metrics for the month of June 2016, including accidents, emergency incidents, ergonomic assessments, training assignments, equipment inspection status, corrective action reports, audits and inspections, and significant safety achievements.

Significant Safety Achievements- 6/16 • The WPC Activity Lead Self-Assessment project final report has been completed and submitted to OCA. • Many of the Level 3 Work Activities are currently undergoing their annual review and re-approval. • Efforts continue closing out “conditionally approved” and “failed” electrical equipment. Additional old equipment is being pulled from the B70 hallway cabinets. • An old chemical clean-out was completed in 70-215. • An ETA Safety Alert was issued regarding safe operation of laboratory ovens. • A new Work Activity was developed for Qualified Electrical Worker Level 2 work in the B60 Air Conditioning Test Chambers. • A new Work Activity was developed for the 70-278 Micro-Balance operation. This will better connect the static eliminators used with Radiological Work Authorization 1227. • An electrical equipment inspection was completed on the 70-114 glove box. It passed inspection and has a green AHJ sticker affixed. • Draft training slides were developed for internal review/comment regarding: • New Hire Safety Orientation • Lab Safety Lead Orientation • Activity Lead Awareness
Short Term Safety Outlook 6/16 • Start the ETA Electrical Equipment Self Assessment project. This will be the third and final assessment for FY16. • Complete the annual renewal of 20 pending Level 3 Work Activities. All require EHS collaboration. • Complete development of awareness training for: • Activity Leads • Lab Safety Leads • New Hire Safety Orientation • Assist with the installation of a new glove box in 62-246. • Coordinate an electrical equipment inspection of the 62-312 glove box. • Perform an old chemical clean-out of the 62-312 and 62-314 lab areas. • Continue to address all remaining ETA equipment on the electrical equipment “conditionally authorized” and “failed” lists. • Complete removal of old equipment from B70 hallway cabinets. • Conduct quarterly SAA hazardous waste inspections of ETA lab areas.
