FloridaHATS • FloridaHATS brochure and rack card • Promote multiple education and training products • 10 Steps to Successful Health Care Transition • Available for distribution in November 2010 • Statewide workgroup of physicians developing condition-specific clinical care guidelines for transition • In collaboration with Florida Pediatric Society • Beginning January 2011 • Submit legislative proposal in 2011 session • Formalize FloridaHATS as state office within CMS • Authorize CMS to serve clients up to age 26 (current age limit is 21 yrs) • Selected as a priority issue for the Hillsborough County legislative platform

HillsboroughHATS • Coalition Coordinator: Marybeth Palmigiano • Coordinate meetings/communication for implementation of action plan • Provide workgroup administrative support, research and other tasks • Vision “The HillsboroughHATS Regional Coalition works toward improving the transition process for youth and young adults with chronic-complex health care needs in the 21st century. Through enhanced patient and family-centered goals, the coalition will work to support a continuum of care, while improving accessibility, throughout the patients’ life span. With the use of education and training, adequate funding, and advanced technology, it is our goal to enhance the patients’ and families’ quality of life for many years to come”

HillsboroughHATS • Action Plan for Hillsborough County • 3 overarching strategies • What is needed for successful health care transition? • How can we effect policies? • How can we ensure better collaboration across multiple systems? • 13 objectives and 40 activities/action steps • Identify local services and build relationships among providers • Identify/ disseminate educational materials for youth, families, providers • Develop a mentor/advocate model for transition • Establish transition guidelines and algorithms for pediatric/adult providers • Encourage providers to adopt transition policies at the practice level • Identify needs for advocacy and policy change • Educate the community, payers and legislators about needed policy changes • Coordinate with schools through IEPs and classroom curricula

School-Based Education and Training Workgroups 5 and 6 are dependent on new AAP algorithm/toolkit and FloridaHATS clinical care guidelines that are forthcoming in 2011, so can wait until those materials are introduced: 5. HillsboroughHATS • Workgroup Co-Chairs • Invite other community members to join for specific activities • Develop meeting agenda and lead discussion • Prioritize and assign tasks, establish timelines • Coordinate through Marybeth • Communication/Meeting Venues • Teleconference (USF service) • Google Docs (share documents; real-time online collaboration) • Email • Any other creative mechanism you can devise!

13. HillsboroughHATS • Workgroups meet independently over next several months • Marybeth will coordinate issues across workgroups, as needed • Janet will keep you updated on FloridaHATS activities; tentative statewide teleconference in December 2010 • Full Coalition will reconvene in April or May 2011 • Report on workgroup activities • Assess progress with plan • Revisit/revise objectives or activities, if necessary • Small budget remaining for local activities through May, approximately $2,000 - $3,000
