Cold chain capacity analysis & EVM for new vaccines introduction Geneva, Oct 2012 S. Kone, IVB/EPI
Both cold chain capacity & EVM are essential for NUVI CCL deals with the physical status of the infrastructure to store and transport the new vaccines EVEM looks at the quality of vaccine management processes to ensure that new expensive vaccines will be handled and used efficiently Cold chain & EVM in support to NUVI
Suggested checklists: CCL: Available/planned capacity stated (yes/no) Supply parameters defined (yes/no) Volume of vaccines estimated, including NV (yes/no) Capacity gap identified (yes/no) EVM: Assessment conducted within 36 months (yes/no) EVM report available (yes/no) EVM improvement developed (yes/no) Improvement plan addresses gaps (yes/no) Assessing CCL & EVM for NVI
Vaccine cold storage estimations • 1. Total annual volume of vaccines: • = ∑n(Packed volume x No. of doses x wastage factor) • Supply intervals, • Safety stock • Grossing factor, • Equipment layout Gross capacity ~ 3.5-4.2 time net capacity • Grossing factor, • Equipment layout 2. Available net cold storage capacity: = Measured net storage capacity or Total gross storage capacity = ----------------------------------------- Grossing factor
Amount of supplies needed vaccines injection equipment distribution plan (routes!!) Capacities needed storage (cold and ambient) cooling/freezing packs (transport & vaccination) transportation (shipping containers, vans) vaccination force (teams, staff) Amount waste generated empty vials/ampoules used syringes Planning tools - Output
Central cold storage analysis-1 • Central/national store: • Specify existing equipment (type, No.) • State the total gross cold storage capacity • State the total net cold storage capacity • Analysis: • Estimation on the total net annual volume of vaccines • Explain how existing cold storage capacity can accommodate this volume • Any of the following is valid: • frequency of shipments (but not >4/year) • additional capacity (within 6-12 months prior introduction) • Lyo. vaccines @ -20°C • Sub-national stores as primary (with clear explanation on how NRA will be involved)
Analysis of SN/district cold storage • For the first year of introduction: • Indicate number of stores with or w/o adequate capacity • Describe actions to be taken to fill the gap 2012 2015 • For the last year of introduction: • Indicate number of stores with or w/o adequate capacity • Describe actions to fill the gap: • No. of cold chain equipment • Redistribution plan
Exple: Additional CC equipment needed • Total needs by 2015 at sub-national stores: • xxx refrigerators (MK404) • yyy WICR of 15 m3. • zzz WICR of 30 m3 • Note: • Number of additional equipment could be procured and installed in phased manner • Selection should be based on the national policy and the availability of energy source
E9 Information systems & supportive management E8 Vaccine management E1Pre-shipment & arrival E2 Storagetemperature E7 Distribution E6 Stock management E3Capacity 80% E5 Maintenance E4 Buildings, equipment, transport Nine criteria for EVM All 9 criteria scores at least: EVM sets minimum standard for immunization supply chain Each criteria includes both hardware-related and management-related indicators
9 global criteria with requirement and set of indicators Identify strength and areas for improvement Guide development and implementation of improvement plan EVM results – tool for communication • 7 general categories for action improvement • Identify strength and areas for improvement • Direct actions for improvement
80% target is set globally as satisfactory level Depending the magnitude of gap, countries may set mile stones towards the target Identify for each criteria indicators with poor score for action Priority to critical indicators, i.e. with high weighted scores. Those will have maxi impact on overall score EVM scores – guidance for improvement Criteria & categories Supply chain levels
EVM scores & guidance for improvement • EVM improvement plan: • Set priorities from criteria • Define activities from categories • Develop budget & implementation schedule • (current) EVM scores present the situation in the past! • Improvement plan provides ways towards satisfactory performance • Implementation of the improvement plan will realize a better future
Network of EVM experts of immunization partners Supportive supervision Quality improvement Training plan EVM assessment Follow-up (to address gaps (Identifies strengths assessments identified) and weaknesses) Technical assistance Investment Implement (As necessary) After minimum improvement Evaluation and planning of 12 months plan EVM – A continuous process of quality improvement EVM a good tool for NVI
