  3. From the Inquiry Menu, select option I Provider File. Then, enter an “X” in the PCH/FCH field, and enter the county number of the facility you are searching for.

  4. This is the first page of the PCH/FCH listing for this county. You can page forward to see additional listings by pressing your F8 key.

  5. Facility numbers are listed here. If a PCH is not found on this listing, it is not a DMS licensed facility.

  6.
