WORKING OF A PANEL STATION ESP. IN CONNECTION WITH THE SCPD DERAILMENT SCPD IS A ROAD SIDE STATION WITH 103 ROUTES. THE BASIC OPERATION IS LIKE ANY OTHER PANEL OPERATION DERAILMENT TOOK PLACE OF 13152 WHILE TRAIN WAS MOVING. IT WAS A CASE OF 2 ROAD CONDITION UNDER MOVING WHEELS. A VERY RARE THING AT THE PANEL STATIONS.
FACTS OF THE CASE • 13152 WAS TO RUN THROUGH FROM SCPD BUT PLANNING HAD TO BE CHANGED AS 14002 SAMJHAUTA EXP. WAS TO PASS VIA JRC. 14002 IS NOT SUPPOSE TO STOP ENROUTE. IT IS CONTROLLED BY DIFF. CONTROLLER. • 13152 WAS RUNNING BEFORE TIME BY 28 MTS AND 14036 WHICH WAS FOLLOWING 13152 WAS RUNNING LATE. SO,PRECEDENCE AND ROOM AT JRC LED TO CHANGE OF PLANNING. • ROUTE WAS CANCELLED AND RECEPTION WAS PLANNED AT THE LOOP LINE AT SCPD. IN THE MEANWHILE M/LINE CONTROLLER SAID 14002 IS NOT COMING 13152 CAN BE TAKEN AT JRC. SEC/CNL TOLD THIS TO STATION AND STATION APPRISED THAT RECEPTIN HAS BEEN PLANNED AND TRAIN SHALL GO AFTER 2 MINUTES. S/ CNL SAID OK LET IT GO.
WHAT HAPPENED AT THE STATION • TRAIN WAS BEING RECEIVED AT LOOP LINE AND ENGINE AND 1ST 8 COACHES CAME ON LINE NO. 2 BUT 9TH AND 10TH COACH DERAILED AS THESE WENT ON LINE NO. 1. • POINT GOT OPERATED UNDER MOVING WHEELS. • IT WAS THOUGHT THAT POINTS WERE OPERARTED WHILE TRAIN WAS MOVING. • NORMAL CANCELLATION ,IE, EUYN WAS DEFECTIVE AND KEY OF THE ESM WAS ALLEGEDLY REMAINED INSERTED ON THE PANEL • PERMANENTLY.EUUYN,IE, EMERGENCY ROUTE RELEASE BUTTON WAS OPERATED AND THE POINTS GOT OPERATED.
CONTD…… • RECORD KEEPING WAS POOR AND DUE TO THIS FACT ALL THE BLAME WAS COMING ON TO TFC STAFF. REGISTERS OF COUNTERS WERE NOT FILLED UP FOR 4 MONTHS AS THE STAFF WAS BUSY IN NI. • COUNTERS WERE SHOWING READING INDICATING HUGE USAGE ,IE, 700 TO 1600 TIMES IN 4 MONTHS. • RECORD WAS CHECKED AGAIN AND IT WAS FOUND THAT FEW COUNTERS WERE JUMPING AND THE ASMCHARGE DIARY WAS HANDY IN PROVING THIS. • EACH MOVEMENT IS CAPTURED IN THE DATA LOGGER AND IT WAS ANALYSED IN VERY MINUTE DETAIL. • IT WAS FOUND OUT THAT ASM HAD ONLY 266 MILISECONDS TO OPERATE 4 BUTTONS TO CHANGE THE POINTS UNDER MOVING WHEELS. IT WAS PHYSICALLY IMPOSSIBLE AND THEN IT WAS AGRRED BY S&T DEPTT. THAT FEED HAS COME INTERNALLY. IT WAS INVESTIGATED AND WAS FOUND THAT THERE WAS WIRING ERROR. • SEAL OF THE EMERGENCY POINT OPERATION BUTTONS WAS INTACT.
CONTD… • PRIMARY CAUSE OF DERAILMENT WAS WRONG WIRING FROM CONTACT D4 OF RELEY NO. 130 NWKR TO CONTACT D5 OF RELAY NO. 135 RWKR WHICH GAVE FEED TO A CIRCUIT AND THIS OPERATED POINT NO. 135 AAND IT GOT SET FOR LINE NO.1 WHENEVER EUUN WAS OPERATED. • ONE EXTRA WIRE WAS AVAILABLE IN THE FEED CIRCUIT LEADING TO THE REVERSE OPERATION OF POINT NO.135. THIS PARTICULAR WIRE WAS SUPPOSE TO EXTEND FEED FOR OPERATION OF DN CALLING ON SIGNAL FOR LINE NO. 1 & 2. CALLING ON SIGNALS FOR LINE NO. 1& 2 WERE NOT OPERATIONAL AT THAT TIME.UNDER THE PREVAILING CONDITIONS POINT NO. 135 WAS MOVING TO REVERSE POSITION UNINTENTIONALLYOR AUTOMATICALLY UNDER CERTAIN CONDITIONS.
LESSONS LEARNT • POOR RECORD KEEPING. • FAULTY PANEL DESIGN THAT THE ASM CAN’T SEE THE COUNTER NUMBERS FROM HIS SEAT. • PERMANENT USE OF EUUYN WITHOUT PROPER REASON WITH THE CONNIVANCE OF S&T STAFF. • NO ENTRY OF FAILURE OF EUYN OR OTHER FAILURES IN THE SIGNAL FAILURE REGISTER. • CONTINUOUS POOR SUPERVISION BY ONE AND ALL.LACK OF COUNSELLING. • POOR KNOWLEDGE AND HALF BAKED INSPECTIONS. • OUT 7400 STATIONS 4300 HAVE ALREADY BEEN CONVERTED TO PANEL OR RRI, KNOWLEDGE IS A MUST.
