What Linkages looks like today • Co-location of CalWORKs and Voluntary Child Welfare Services (CWS) • All voluntary CWS cases are screened for CalWORKs eligibility • Cases with mutual customers are identified as crossover cases and referred to an integrated team • Increased communication
What’s working…. • Awareness of resources and strengths • Reduction of barriers and service duplication • Sharing of information, advocacy, and appreciation
Why Linkages works… • Emergency triage team • Coordinated Case Plan (CCP) with Multi-Disciplinary Team within the first 30 days • Referrals to Behavioral Health (AOD, MH, DV) to establish appropriate funding streams • Continuum of care
Where we want to expand Linkages… • Family Reunification • WTW Sanction • CCP’s at 6 months and case closure • TANF intake
Destination FR… • Identification and screening process • Assignment process • Team Decision Meetings (TDM) • WTW participation requirements included in recommended court ordered case plan • Case Transitions at Child Welfare closure
Destination Sanction… • Identify population • Address causes and find solutions to cure • Refer and transfer to integrated caseload
How outcomes are measured • Current measurements -Percentage of cases screened for program eligibility -Percentage of initial CCP’s • Future measurements of new populations -Percentage of screened cases and total CCP’s completed -Reduction of CWS recidivism -Increased WTW participation
