This project aims to increase access to ocular care services by improving utilization rates and providing preventive services to the target population with identified disparities. By increasing the number of ocular providers, expanding clinic infrastructure, and implementing innovative strategies like electronic referrals and ROBO calls, the project seeks to enhance diabetic eye exams and improve HEDIS markers. Continuous quality improvements include collaboration with internal medicine for screenings, optimizing patient flow, and reducing registration and wait times.
