National Family Caregiver Support Program: Services and Eligibility

The National Family Caregiver Support Program provides information, assistance, counseling, training, support groups, respite, supplemental services, and forms and assessment tools for eligible caregivers. Eligibility criteria include age and care recipient's age or condition.

  2. Caregiver Eligibility • Someone, age 18 and older, caring for a person who is age 60+ • Someone, age 18 and older, caring for a person with Alzheimer’s or related disorders, dementia, at any age

  3. Older Relative Caregiver Eligibility • A relative (not parent) who is age 55+ and is caring for a minor child(ren), age 17 and younger • A relative, including parents, who is age 55+ and is caring for an adult, age 18 to 59, with a disability

National Family Caregiver Support Program Services • Information: Public education, brochures, posters, general information relative to the program and to resources to support caregivers and their families. • Assistance: One-to-one contact. This service may include assistance to the caregiver in locating and arranging for services from a variety of agencies. Also addressed is the assessment of the needs of the caregiver and care coordination. Included is the development of service plans, assessment and re-assessment. • Counseling/Training/Support Groups: Training is implemented to provide advice, guidance, and education, etc., to caregivers in individual or group settings. Support Groups for Caregivers to share their Caregiving experiences and talk. Counseling may only be done by a licensed counselor to be billed to the program.

National Family Caregiver Support Program Services (continued) • Respite: Child day care, In-home services, adult day care, or if need be, an overnight stay in a residential setting which might be a nursing home or assisted living. This service is defined as “intermittent” and is intended to be limited. • Supplemental Services: Supplemental services include adaptive equipment (hand rails, ramps, lighting, etc.), loan closet items, or meals and transportation for the caregiver. Also included are car repairs, smoke detectors, homemaker and chore services, personal care, or other services approved by the Division. The $400 limit per year, per client for home modifications and durable medical equipment (DME) can be increased with Division approval.

National Family Caregiver FormsAnd Assessment Tools • Aging Needs Evaluation Summary (AGNES) needs to be done for Care Receivers (loved ones), annually and quarterly, whose Caregiver has asked for Respite and Supplemental Services. • Service Plan needs to be done for all Caregivers, noting services needed. This plan needs to be signed by the Caregiver (client) and the Access Care Coordinator. • A nursing assessment is required if the ACC determines, via the AGNES, a need for personal care. This assessment is required to be done by a licensed nurse, and if a LPN did the assessment, a RN or physician must also sign. Access Care Coordinators who are not licensed nurses are not qualified to do this assessment.
