2. Total severity 4 and 5 observations for the month of Oct.’10:- 37 Nos.

3. Severity 4 :- 100 T crawler crane was marching with load on an inclined surface.(Tata Steel LD#3) (Model-II)_ East Action Taken:-Activity was stopped & decided to shift material in part-wise till trailer deployed.

4. Severity 4 :- Man Basket was being used for erection work.(CSP) (Model-III )_ East Action Taken:-Activity was stopped & Man Basket discarded.

5. Severity 4 :- “C” channel was kept on the purlin without securing. (Suraj Logitix_Jamshedpur) (Model-III)_ East Action Taken:- “C” channel removed.

6. Severity 4 :- Manual splicing sling was being used for gable end column erection.(Jaypee) (Model-III)_ East Action Taken:- Sling discarded.

7. Severity 4 :- Short height vertical ladder without fall arrester was being used for access to the after.(Jaypee) (Model-III)_ East Action Taken:-Ladder height extended 1 mtr. above from column also fall arrester provided.

10. Severity 5- Electrical Bare wire found on roof without plug sockets .( Ampacet- Empire) (Model-III) _West- MAHA. Action Taken:- Activity stopped immediately. Explain importance of Plug &Socket & same was provided.

11. After Before Severity 5- Scaffold Wheel found with out Locking arrangement . (Ampacet-Empire) (Model-III)_West Action Taken:-. Wheel with locking arrangement provided immediately.

12. After Before Severity 5- Electrical wire routed throughout ground. (Ampacet-Empire.) (Model-III)_West-MAHA Action Taken: Wire routed through 3mtr up by Pipe immediately.

13. Severity 5- GAS Cutting Set found w/o FBA at Church gate Extn. work.(Church-Gate Extn.- Sai Project.) (Model-II) _West Action Taken: Asked Builder to provide it immediately. (Closed)

14. Severity 5 – Oxygen Cylinder used for shifting primary structure Manually.(ChurchGate Extn.-Mumbai-Sai Project)_ (Model-II) West -Maha Action Taken:- Asked Builder staff provide 6inch pipe pieces for same.

15. Severity 4- Improper access at Indo Euro & rigger found w/o PPEs (Indo Euro- Suryodaya.) (Model-II)_West MAHA Action Taken: Activity stopped and Asked Builder to provide scaffold as per checklist. (Closed)

18. Severity 5- Electrical bare wire used & wooden Board w/o Weather protection.(Jindal SS -R.K. Builder) (Model-II) _West _GJ Action Taken:- Work was stopped and asked to changed the board & provide proper plug top.

19. Severity 5- Riggers was descending by using Girts.(Siemens- Mundra) (Model-II)West- GJ. Action Taken:-Work was stopped & Boom Lift Provided Immediately.

20. Severity 4- Hole Bari was just fixed instead of bolt. (Siemens –Mundra) (Model-II)_West –GJ Action Taken:- Work was stopped & hole bari removed and proper connection was done.

21. Severity 4- T-Rafter were erected w/o Life line Pole . (Welspun –Anjar) (Model-III)_West -GJ Action Taken:- Work was Stopped & corrected immediately.

24. Severity 4-: Worker found standing and working on girts and hooking his safety harness to a clit- Perlos (Model-II)_TN Action Taken: Activity stopped and instructed supervisor to monitor closely while workers are working at height and to provide lifeline and proper access.

25. Severity 4-: Worker found standing on scaffolding pipes and working –Perlos (Model-II) _TN Action Taken: Activity stopped and instructed supervisor to provide working plat forms.

26. Severity 4-: Workers found working on non standard scaffolding and using single lanyard safety harness – Sheenlac (Model-II) _ TN Action Taken: Activity stopped & instructed supervisor to provide working platform and ensure that the workers are using standard PPE. (Closed)

27. Severity 4-Electric wires found directly inserted into the distribution board . – Sheenlac (Model-II) _ TN Action Taken: Activity stopped & asked to procure standard distribution board and to used plugs.

28. Severity 4-: Workers found working at height without any fall protection– Sheenalc site (Model-II) _TN Action Taken: Activity stopped & PPEs provided.

29. Severity 4-: Loose sheets found unattended on the roof – Sheenlac site (Model –II) _TN Action Taken: Loose sheets and other material secured and asked builder site supervisor to maintained same.

30. Severity 4-: Column erected on footing without proper supports .- Sindhuja (Model-II) _TN Action Taken: Activity stopped & Colum secured by four guy rope and instructed supervisor to follow safe erection methodology.

31. Severity 4-: Material found shifted without guide ropes- Sindhuja (Model-II) TN Action Taken: Activity stopped & instructed supervisor to used guides ropes while shifting material

32. Severity 4-: Worker found not anchoring safety harness to a fixed support & area below sharp edge materials found. - Perlos (Model-II) _TN Action Taken: Activity stopped. Life line provided &housekeeping done and to barricaded the area .

33. Severity 4-: Below are is not barricaded while height work is in progress - SCL (Model-II)_TN Action Taken: Activity stopped & instructed supervisor to barricade the area whenever height is in progress
