Hire Slip – Badge Request Form We have combined the information from the old “ Contractor Hire Slip” and “Background Screen Verification” forms into this one “Hire Slip-Badge Request Form”. You no longer have to provide 2 separate forms at the badging office. Documents now required for badging at the Dow Chemical, Texas Operations, Freeport site: Updated Hire Slip-Badge Request Form - The original, completely filled out, signed form, must be brought to the site badging office by the individual. The forms cannot be faxed or e-mailed in. Training badge from the safety council - must be brought to the site badging office by the individual. A form of government issue picture ID (drivers license, passport, etc.) - must be brought to the site by the individual. Badging office hours are: 6:30 AM - 4:00 PM (B-1210) Contract employees must have their site access badge and safety council training badge on their person at all times when on site. ICS: Scott Davis @ EOB shared a concern over the walking surface at Gate B-40 employee walk in entrance. It was escalated to a site level & within 48 hrs the repairs were complete. – Recognized By Eileen Linnett • Miken: George Velez @ Performance Packaging Assets has worked extended hrs to have job ready for multicraft the next day. George was also award the employee of the month for the plastics department @ B-2700. – Recognized By Charlie Hill • Miken: Scaffold crew @ A-1300 August 2013 T/A did multiple slicker suit and acid hood jobs in the CDC area and did a great job of rotating their people to address the heat. They did their jobs with a safety first attitude. – Recognized By James Wheeler • Miken: Virginia Flores @ B-3900 helped with a Safe Work Permit self assessment and was very knowledgeable about her craft and the permit. She asked good questions and took the permitting process seriously.
