FIU Premier Care Program Offered by the FIU Academic Health Center HealthCare Network and the FIU HEALTH Faculty Group Practice
Program Description An advisory program available to provide guidance to individuals requesting assistance with: • Navigating through the health care environment and health insurance world when faced with the need to make decisions regarding care when they or a dependent have a new or chronic disease or condition. • Learning about available health insurance plan, and University resources and/or • Transition of care planning for individuals changing their health care provider when they have a new, chronic or serious medical condition.
Purpose • This advisory program, provided by the FIU HealthCare Network (FIU-HCN) and the FIU HEALTH Faculty Group Practice staff in conjunction with the FIU Human Resources Department is available to the FIU community at no cost and on a personalized basis, respecting privacy and the needs of each individual.
Collaboration • The FIU-HCN and FIU HEALTH Faculty Group Practice health care professionals will work collaboratively with the FIU Human Resource Department representatives as well as key contacts from each of the University sponsored health insurance plans; Blue Cross/Blue Shield, Coventry and AvMed.
Objectives • To provide guidance during the annual open enrollment period, during the early change period following the selection of a new University health insurance option as well as throughout the year as the FIU Community has the need.
Objectives Continued • To provide care coordination assistance to established patients (faculty and staff/the FIU Community) who are under the care of a physician at the FIU HEALTH Faculty Group Practice in response to: • A change in their health insurance plan • Transition of care/care coordination needs (including authorization) when transitioning care from a community provider to a Faculty Group Practice provider • The need to acquire medical records from their community provider(s) to ensure care continuity as well as pharmacy needs related to maintenance medications and chronic health care management
Objectives Continued • To acquaint faculty and staff with available medical care and provider options on the Modesto Maidiquecampus such as the FIU HEALTH Faculty Group Practice and through Herbert Wertheim College of Medicine physicians.
Limitations To uphold privacy laws, the FIU-HCN and FIU HEALTH Faculty Group Practice staff cannot contact an individual’s physician or other provider of care about medical care, bills and/or authorizations unless you are a patient of the FIU HEALTH Faculty Group Practice.
Human Resources Role • The Human Resources Department will continue to be the primary resource related to health plan benefit questions.
