  3. 9 CHARACTERISTICS OF WELL-ORGANIZED AND MANAGED PROJECTS Indicator of Success Criteria for Gauging Progress Toward Goal • Program has a clear and focused vision and mission. • Program goals and objectives are realistic and measurable. • Stakeholders, including community partners, family member representatives and front-line staff, • are involved during program development and implement. 1. Developed a Well-conceived Plan 2. Established Partnerships with the Juvenile Justice System and Health Care System • Grantee is experienced in working with the Juvenile Justice and Health Care Systems 3. Collected and Maintained Data • A systems for collecting and reporting program information is available and utilized. • Program is supported by youth and family serving agencies, including CBO’s, faith-based • organizations, and public service agencies 4. Community Support/Network 5. Grantee Involvement • Grantee is the lead agency, actively providing direction and • coordination for the project. • Grantee involvement and support are continuous. 6. Connected Workforce Development, Juvenile Justice, and Health Care Systems • Grantee coordinates with and utilizes resources available • through the Workforce Development, Juvenile Justice, and • Health Care Systems. 7. Leveraging Resources Through Collaboration and Partnerships • Project effectively identifies and utilizes other resources and • funding streams to support project goals. 8. Continuous Improvement • Project conducts self-assessment and actively seeks and accepts • available technical assistance. 9. Shared Leadership Information Sharing • Decision-making and information is shared with stakeholders.

Project • 96% Returned to and/or remained in school • 96% Achieved completion of occupational skills training in demand occupation • 100% Completed work readiness education and work experience • 100% Improved basic skills by at least one grade level • 89% Obtained a JOB! • 100% Retained Employment for Six (6) Months
