Measures • Increase participation of Residents in walking program. • Decreased number of falls with injuries. • Decreased number of falls. • Increased Resident/family awareness of need to purchase preventative devices such as hip protectors. • Increased Resident participation in exercise programs. • Increased staff participation in solutions to address complications from a Resident fall through the Post Fall Huddles. • Identification of Residents at greatest risk to injury from falls, ie. Osteoporosis, Dementia diagnoses, poor nutritional intake, sudden undesirable weight loss. • Reinforcement of measurement and assessment tools already in place. • Analysis of data in order to identify needed areas of focus.

Lessons Learned on Sustaining Falls Improvement Work during Action Period When I first became aware of the LLSF, I believed that it would offer an educational opportunity for all staff through webinars. After being discouraged, I contacted Nadine, who provided me with the direction needed. The program is designed to build upon already existing structures such as initial and ongoing assessments. However the data interpretation is a strong indicator of where you need to go. For us it was increased awareness of prevention through the walking program, increase Resident/ family/staff awareness of preventative measures such as hip protectors and exercise program participation, and how to ultimately get staff participation and awareness increased by simply have a post fall huddle and documented this information for reference to assist in future occurrences.

  8. Challenges to Sustaining Falls Improvement • Barriers we a lack of staff members being able to participate. Our small facility size reducing the amount of time staff are able to participate and it falls on the lead to make initiatives at team meetings and during educational opportunities. • Facilitators include education and participation of Residents, families, and staff in existing programs and awareness of the preventative benefits of participation. • Ongoing progression with education to the Residents, families and staff.
