  3. AIM To incorporate the use of a falls prevention model; BEEEACH for high risk clients To Apply BEEEACH strategies for high risk clients through the use of individual care plans 100% of clients with documented falls will have a prevention and or injury reduction plan

Measures Use BEEEACH falls prevention model on all high risk clients and clients with documented falls Refer to appropriate disciplines for assessment Implement care plans using BEEEACH strategies Audit charts monthly to ensure 100% high risk clients have documented care plan for falls prevention Link to other programs: Elders in Motion, Remember When Constantly Review process, Work with the Team, Staff and Clients for continuous feedback

Lessons Learned on Sustaining Falls Improvement Work during Action Period Need to make our Falls Team a priority (too easy to do other work, causing us to fall behind) Maintain the time and commitment we agreed towards the Fall Team, maintain weekly meetings Use of the Morse Falls Scale is being used on all admissions with 100%, key tool for our Home Care criteria

Challenges to Sustaining Falls Improvement B: New Program, started from scratch Staff turnover Multidisciplinary Team with use of different language, terms F: Good Team, productive when we meet Use of PDSA’s helped us with planning, moving forward **Use BEECH tool for Care Planning** **Continue to use PDSA worksheets**
