  2. Background • Squadron CO is causal factor in numerous Aviation Mishaps -- SUPERVISORY ERROR • Why?? • CO ultimately responsible for safety of his people • CO has the ability to get his people the help they need

  3. Purpose of Councils • Identification of Hazards • Determination of the root causes • Resolution of the issues • Reduce Human Error to acceptable levels

  4. The HFC shall review personal and professional circumstances, and direct particular attention toward uncovering underlying medical, physiological, social, behavioral and/or psychological factors which could adversely affect aircrew or maintainer performance. • The HFC is convened only in the interest of aviation safety and shall make no recommendations which are disciplinary in nature.

  5. MAINTENANCE CO XO SO GSO MO AMO QAO Flight Surgeon HFC Core Members • AVIATION • CO • XO • SO • ASO • OPS • TRNG • Flight Surgeon

  6. CONOPS • Held Quarterly and as needed • Dictated by Squadron Tasking • Compartmentalized • Det OICs/Det MOs/Det LCPOs brief the council alone • Other Det/Workcenter Leaders not present • CO only one allowed to take notes. Retained in CO safe and destroyed when CO transfers. • NOBODY ELSE HAS ACCESS TO THE DATA

  7. Hazardous Traits • Below average new guy • Overconfident or Experienced Aviator/Maintainer • “Best Stick” or “Best Mech” • Consistent Poor Performer • Overstressed Aviator/Maintainer
