Includes all required supplemental forms and signatures. • Applications can’t get lost in the mail. • Additional production time each week to meet bonus requirements. E-apps uploaded by 3:00 p.m. central time on Friday will be entered for submit advance the following week. • Branch Manager receives an e-mail each time an application is uploaded by his branch.
When do I have to use a paper app? • If the E-App is not currently available for your state • For the Hospital Income Plan, Dental, Vision and Auto Club • ALX in South Carolina • ALX applications on payroll deduction mode • For special applicant / owner situations such as guardian, business owner for key man insurance, etc.
Who can be the applicant on an E-App? • Insured • Parent or Grandparent - for a child through age 20 if the child is single and lives in the parent’s or grandparent’s household or is single and attends college • Spouse – for amounts through $50,000, a spouse may sign an application on the life of the other. No application is to be written without the knowledge and consent of the proposed insured.
Applicant Information • The applicant information is always entered in section A. • If a mother is applying for coverage on a child, the mother’s information must be entered in A. The information on the child is entered in section B. • If a husband is applying for coverage on his spouse; the husband is listed as the applicant in section A. Use section B for the wife
Troubleshooting • Vista user account must be turned off and remain off for the E-App and proposal to work correctly. • The proposal system and E-App currently will not work correctly on Vista operating system with a 64 bit processor (see e-App minimum requirements). • Keep your user id and password active by signing into the website on a regular basis. • Make sure you download the latest version of the software as instructed.
Primary Agent Information Make sure the Agent has the correct Branch/Agency set up on his primary Agent screen. If this is set up correctly, the Branch Manager receives an e-mail when an Agent uploads an application.
Primary Agent Information Change cell phone numbers or agent e-mail addresses on this screen.Add partner agentsIf you have signed applications on your laptop, you can add an agent for commissions.You must upload all signed applications before you change any information on the primary agent.
Uploads Assign commissions to the correct agent before uploading.All signed apps must be uploaded each week. You cannot hold applications.Train agents to avoid multiple uploads. Check e-mails before uploading a second time.
Laptop Presentation and e-App • Sponsorships are loaded into LMS when an e-App is uploaded. • The recommended plan and amount will automatically load on the application based on the customer’s need. • A Final Expense need will create a Whole Life application. • A Monthly Income or Education need will create an ART application. • A Mortgage Need will create a Mortgage Decreasing term application • For accident and health needs, the application will automatically load. The agent selects the type (individual, single parent, family) and amount or plan of coverage.

Submitting LNL Paper Applications • Make sure all application information is complete and correct before you submit the application to the Home Office. • Incomplete apps slow down the policy issue process.
A-250 Application Importance of the image ID number Don’t staple. Don’t mark around the numbers. Don’t copy the applications.
Application Use black ink only List your LNL Agent number, insured’s name and address, sex, age last birthday and date of birth Record the name and relationship of the beneficiary and names, Social Security numbers and date of birth for their spouse and eligible children Be sure to ask the replacement question and comply with all replacement rules and regulations
A-250 Application The A-250 application is used to apply for all Life plans except Group Term and Career Life Plus In South Carolina use A-260 to apply for Modified Life (ALX)

A-250 Application Space for beneficiary, contingent beneficiary, owner and contingent owner Choose the plan in #12. If it is a MDT policy the interest rate and number of years of the loan is required. This block also contains space for the face amount and amount paid with application.
A-250 Application Add riders in #14 Question #15 is about citizenship. If the proposed insured is not a citizen, you must complete form A-282-2 and submit it with the application.

The effective date of policies issued by the Company from the A-250 application is the date printed on the policy and is based on the mode of premium payment. Policies issued during the first 15 days of the month will be dated the 1st of the current month. Policies issued after the 15th will be dated the 1st of the following month. The proposed insured may have up to $50,000 coverage prior to the effective date based on the provision in the conditional receipt. Effective date of coverage vs. policy effective date
If an application fee is submitted with the application, the applicant has coverage from the date of application based on the Conditional Receipt (including bank drafts). If an application fee is not submitted with the application, the proposed insured will not have coverage until the effective date of the policy and only then if the first premium is paid in full. The rule for payroll deduction policies is as follows: The company considers coverage under payroll deduction applications to begin when the application is signed. Coverage is only effective under payroll deduction applications if: the proposed insured is an acceptable risk to the Company for the insurance applied for at Standard rates (as to employee applicants for Career Life Plus, the employee will be covered for the amount for which he or she would medically qualify); and a payroll authorization (if required) is completed and attached to the application. If authorizations are not required by the Company, a franchise number must be included on the application. It is not necessary for he first month's premium to be submitted with the application. Cancer/Hospital Intensive Care Applications • You must complete the premium information on the back of the application and indicate the total amount to be drafted. • If customer chooses both the Cancer and the Hospital Intensive Care, you must add the two premium amounts together and put that total on the back of the application. 32.00
Oral Fluid (Oral Swab) Authorization Form – • List the Bar Code Number under ‘Additional Comments’ for paper applications. • Not necessary to mail the Company page of the Oral Fluid (Oral Swab) Authorization Form to Home Office. • ALWAYS collect the oral swab specimen at the time the application is completed. • ALWAYS mail the specimen to the lab as quickly as possible. Preferably mail the same day you obtain the specimen, but no later than the following day. • A delay in submission to the lab could affect the accuracy and reading of the test.

Incorrect Ink Color • All applications must be completed in BLACK ink. Any other ink color can cause problems with the imaging and processing of the application.
Checks/Money Orders Collected on Bank Draft Applications • Do not collect checks or money orders on bank draft applications. • The first premium is automatically drafted from the customer’s bank account. • Liberty National does not draft savings accounts, only checking accounts.
Missing Forms • Bank Authorization Form • R-3616 must be submitted • with all applications • submitted on a bank draft • mode – even trial apps.

A-251 Application A-251 – indicate spouse rider and or children’s rider The same questions are on the A-251 that are found on the A-250 A spouse can sign for coverage on their spouse if the coverage does not exceed $50,000.

Agent’s Instruction Guide • Signature Rules • The applicant must sign the completed application in the presence of the soliciting Agent or member of management who must then sign his or her own name as a witness. No blank of partially completed application may be signed by the applicant. No application may be left behind to be signed in the Agent’s absence. • The rules apply to all application forms, amendments, supplements, certificates of good health, reinstatement applications, policy delivery acknowledgments, etc.

Agent’s Instruction Guide • Amount Paid with App • The amount must be at least one full mode premium. The Agent should not accept partial premiums. An Agent cannot pay premiums for coverage on anyone other than an immediate family member. • An Agent should never accept CASH for payment of any premium, including initial application and subsequent periodic premiums.
Agent’s Instruction Guide • Replacement Regulations • When a replacement occurs, the replacement question on the application and the statement in the Agent’s Statement must be checked to indicate a replacement. • Submit to Home Office: • A copy of all replacement forms signed by the Agent and the applicant. • A copy of all Sales Proposals.
