The Professional Portfolio • Biographical information • Provide information about yourself, include name, address, phone number, fax number, e-mail address birthplace and nurse licensure number. • Educational background • List educational program course work you have completed; any degrees earned • Certification • Identify number of certifications you have achieved by name, the certifying organization, certification date, and certification renewal date
The Professional Portfolio Employment History • Describe work history; present a view of your professional nursing career • Describe the full depth and breadth of you journey from novice to expert nurse. • Resume • A one page resume identifies work and educational experience along with certifications, beginning with the most recent in each area/category.
The Professional Portfolio • Competency Record • Obtain & make copies of your most competency record checklist. • Identify new competencies acquired within the last year by highlighting them.
The Professional Portfolio • Personal and Professional Goals • Include copies of your personal and professional goals • Develop five professional development goals annually; evaluate them at the end of the year • Review these goals; & reflect on your professional development and growth through out the year
The Professional Portfolio • Professional Development • List your obvious professional development experiences • Include continuing education certificates for agency, regional, state, or local programs • List and cite nursing and non-nursing articles you read for self-development and other areas of personal improvement are important to list such as leadership development and other areas of personal improvement.
The Professional Portfolio • Presentations, Consultations, & Publications • Identify original presentations developed; individually or with others • Include copies of the original article in addition to a photocopy of the newsletter or journal • Professional Activities • Ensure to document your participation in professional organizations and activities • Identify committees you serve on and the role/position you fill • Community Activities • Include the name of the community organization, committee, or program on which you serve.
The Professional Portfolio • Honors and Awards • Identify and include all honors and awards you have received • Letters • Ensure to include copies of thank-you letters from patients, families, peers, organizations, and others. • Offer examples that reflect your broad range of expertise in the healthcare arena.
