  2. QUESTIONNAIRRES Objective is to help you design questionnaire to facilitate capture and computerization of PETS and absenteeism data to address the objectives of the study. In all surveys, data processing is almost always the ‘ bottleneck’; survey done in 2-3 months, data entry 3-4 months and data cleaning 6 months or more but still ‘unclean’. When properly done: • Neatly filled out questionnaires • Consistency in response codes. • Easy to read for data entry • Easy to analyze. .

  3. How many questionnaires. What determines the number of questionnaires for PETS? • Scope of the study (Tracking funds and/or other aspects of the service). • Administrative structures (Central/federal, Region, Districts, sub-national governments, facilities). • Path for the flow of funds. • Types of funds to track in the sector(capitation, textbooks, construction, salaries ). Can vary from local government to another in decentralized systems depending on autonomy in decision making

Types and number of Questionnaires for PET • Questions are applied to different levels of government and the facilities where services are provided. • Data sheets for extraction of data at specific levels. • Questionnaire with sections covering different aspects (Section 1, 2, 3, 4…) or separated for each level of government to avoid confusing researchers. • Qualitative section of the a questionnaires. Include a sheet or two at the end of the questionnaire where qualitative information and clarification of answers to some questions are recorded.

  5. Levels to apply Questionnaires • Central government (MOES, MOF,etc). The starting point for tracking funds/inputs). • Province/Region. • District. • Sub district. • School/ Health facility.(End point for tracking funds) • Student/patients.

Government Questionnaires: • Refers to Central Government(MOE/MOF), Provisional/Regional /District • Establish financial or in kind transactions that occur at each levels with respect to flow of resources. • What additional resources are added at lower levels of government to resources from higher level of government flowing to sevice delivery points (schools/health centres). • Who contributes to such additional resources(in kind and cash). • Which resources are you interested to track in your PETS. • How are different resources accounted for at school level and other levels.

  7. Facility Questionnaire • Implemented at service delivery point; a place where various aspects of the service provision can be observed; and where all the funds/inputs converge and influence the delivery and quality of service. • A set of questions intended to collect data to address the objectives and issues identified during the PETS preparatory stage. • Most important questionnaire but expensive to execute in terms of time and coverage. • Detailed depending on scope and objectives of the PET.
