2011-2012 MiPHY Student surveying open September 1, 2011 – May 31, 2012 Entirely administered via the web in lab classrooms, coordinated locally Grades 7, 9, and 11 All Middle and High Schools are eligible, including public, public school academies, and private
2011-2012 Survey Middle and High School Versions 175 High School questions 159 Middle School questions Students complete in approximately 30 minutes
MiPHY Quick Start Identify at least one District Administrator and at least one Building Administrator per participating building One person can have multiple access levels for example the same person could serve as the building administrator for more than one building.
For New MiPHY Administrators Confirm or obtain MEIS Account for each Administrator (www.michigan.gov/meis and click on MEIS logo) Submit MiPHY Security Agreement form for each Administrator for each building (www.michigan.gov/miphy or www.michigan.gov/miphyadmin)
MiPHY Access Levels and Role Types(for the Security Agreement) District Administrator Building Administrator Survey Administrator Reports Viewer
MiPHY Access Level 1. District Administrator - The designated person(s) at the district level that registers the district and the buildings in the district. They are also responsible for closing the MiPHY for the district when all buildings are finished surveying.
MiPHY Access Level Building Administrator - The designated person(s) at the building level who administers the MiPHY survey. They are also responsible for closing the MiPHY when the building is finished surveying.
MiPHY Role Type Survey Administrator - The designated person(s) who have the ability to generate daily logins and passwords for the survey application.
MiPHY Role Type 4. Reports Viewer - The designated person(s) who have security clearance to access the summary and graphical reports.

Registration District administrator registers district and buildings (Registration tab on the MiPHY Administration site: www.michigan.gov/miphyadmin)

Sampler District or Building Administrator enters student counts to determine sample size by grade. (Sampler Tab on Survey Administration website)
Random Selection Randomly select students using sampling size, if applicable. (User’s Guide and Guidance Tab on Survey Administration website)
Pre-survey tasks Send out the MiPHY Parental Notification Form (Guidance Tab on Survey Administration website) Schedule lab and survey times for students
Choosing MiPHY Version MiPHY- all risk behavior and risk and protective factor domains Violence Weight and nutrition Bullying Physical activity Alcohol Depression and suicide Tobacco Sexual activity Other drugs MiPHY Basic – the MiPHY survey excluding the suicide and sexual behavior questions
Daily Survey Tasks Obtain daily survey password (Password Tab on Survey Administration website Set up computer lab according to technical specifications (Guidance tab in MiPHY Administration site) Survey students (www.michigan.gov/miphysurvey)
Survey Progress Track survey progress (Survey Progress tab on MiPHY Administration site) Building administrator closes building survey by marking “complete” (Progress tab in MiPHY Administration site)
District Survey Progress District administrator closes district survey by marking “complete” (Progress tab in MiPHY Administration site)
Reports Access reports within 3 business days (Reports Tab on Survey Administration website)
