Benefits form being dementia friendly Increased staff awareness for patients and carers needs. Carers considered at all times and feel more supported. Increase in diagnosis rate risen from 52% to 63% in Oakley and Overton surgery and from 57% to 67% in Tower House surgery. All patients having meaningful personalised care plans
What have we achieved and plans for the future 15 practices in the Wessex region designated as dementia friendly. 15 in training and 28 have been approached Resulting in 900 patients plus their carers receiving an enhanced service Plan to cover all the practices on the IOW and further practices throughout Wessex. Supporting the spread of dementia training in acute hospitals, reducing length of stay and supporting the national plan for dementia training in front line staff.
