PURPOSE Sjogren syndrome is a chronic autoimmune systemic disease characterized by polyglandular tissue destruction, leading to keratoconjunctivitis sicca and xerostomia. To find a way to detect early changes of corneal layers in Sjogren patients can be crucial in determining an earlier diagnosis and the management of such patients. We examined the corneal layers in patients with Sjogren syndrome and correlated eventual changes with the stages of the disease.
MATERIALS Corneal Confocal Microscopy (CCM) was performed in 44 eyes of 22 patients with Sjogren syndrome and compared with a matched cohort of healthy subjects. Mean age: 48.68 ± 11.58 years (range 24 to 72 years) Sjogren Sindrome diagnosis was made on the basis of the classification criteria of the American–European Consensus Group
MATERIALS Every eye underwent Schirmer test with and without stimulation and Corneal confocal microscopy using a Nidek Confoscan 3 device. Illumination was set at 170 for every examination. Corneal optical reflectivity at Bowman membrane and at 50 µm, 100 µm and 200 µm has been measured in both Sjogren and healthy subjects. These measurements have been correlated with Schirmer test with and without stimulation. Student T test has been used to check the significance of differences. Pearson index has beed used to evaluate the correlations

RESULTS The increase in reflectivity at Bowman showed a good correlation with Schirmer test without (R2= 0.59) and with (R2= 0.68) anaesthesia.
CONCLUSIONS According to our findings in Sjogren patients there is an involvement of the superficial corneal layers related to the stages of the disease. Corneal Confocal Microscopy is able to detect early changes of corneal layers related to the stages of Sjogren disease.
