Schindewolf Clinic • The Schindewolf clinic strives to promote good student health, attend to school related injuries and sudden illnesses. The clinic also functions to help prevent & control disease and to educate students, parents & others concerning health related matters.
Medications • ALL PRESCRIPTION MEDICINES MUST BE BROUGHT TO THE CLINIC BY PARENT OR GUARDIAN: • ADHD medicines • Asthma inhalers, prescription allergy medicine • Antibiotics • ANY medicine you get from the doctor or the doctor writes a prescription • Non-prescription medicines (Advil, Tylenol, etc): Must be provided by parent/guardian • May be brought to the clinic by student • ORIGINAL CONTAINER • Dated note from parent • May give for 2 weeks without an MD order
Clinic Emergency Card Please complete and return Include any medical information and CONTACT NUMBERS Emergency contacts are for Emergency Only Only the person who signs the card will have exclusive rights to remove the student for early dismissal, approve others to pick up student or administer medications to the student.
IMMUNIZATIONS Students who are delinquent on immunizations are not allowed to attend school NEW REQUIREMENTS PRIOR TO 7TH GRADE • TDaP within last 5 years • Meningococcal Vaccine • Varicella Vaccine (2 doses) unless proof of disease Be complete and on file by end of 6th grade school year
Health Related Problems • If your child has any health related problem that effects his/her ability to attend class or participate in a school function • Any change in medications that may cause problems or illness • HIPPA TALK TO THE NURSE

Do • Wear appropriate and properly fitted clothing. • Choose clothing that is not distracting. • Do wear school spirit shirts. • Keep hair clean, neatly combed, and out of the eyes. • Limit piercings to earrings and studs.
Don’t • Wear clothing the exposes the midriff or is low cut, see-through, or backless. • Color or style hair in a manner that may be distracting to others. • Wear bottoms that hang on the hipbone or are too short. • Wear pants that are extremely baggy. • Wear all or part of a military uniform.
