Learning points: IP deviated from the set scope of the maintenance IP used non insulated tools Recommended Actions: Always use electrical insulated tools when working on live electrical equipment Never deviate from the pre-set scope of the maintenance and if necessary carry out hazards identification before proceeding Discuss this alert in HSE meetings and Tool Box talks and Distribute to your contractors Post on HSE notice boards Include in your HSE induction What Happened A certified Competent Electrical Person (CEP) isolated the electrical control panel in an attempt to carry out routine maintenance as per maintenance craft procedure The control panel was isolated by turning off the main switch and removing the fuses of the 415 V panel feeder During the maintenance activity the IP decided to clean the live terminal upstream switch using a non-insulated paint brush The metal sleeve of the brush touched the live electrical terminals causing electrical flashover which resulted in burns to the IP Contact MSE511 for further information or visit HSE website Alert No 04 June 2010
