ED - OR - ICU • Attending, Fellow, or Chief Resident/Acting Chief Resident: • Initiates MTP as verbal order to: • ED - Nurse Scribe • ICU - Bedside RN, who notifies the Charge Nurse • Follows up with MTP Form and gives to designated MTP RN Appendix C MTP Process • OR - Circulating Nurse ICU Bedside RN notifies Charge Nurse of MTP OR Circulating RN notifies Charge Nurse of MTP • ED Nurse Scribe: • Uses designated BB phone • to call the BB to unlock • the MTP refrigerator • Gives BB pt. info.: • Name • MRN* • Gender • Notifies OR & ICU of MTP • Remains sole contact with • the BB throughout the MTP • ICU Charge Nurse: • Uses designated BB phone # • to notify the BB of the MTP • Gives BB pt. information: • Name • MRN • Gender • Notifies OR of MTP • Remains sole contact with • the BB throughout the MTP • OR Charge Nurse: • Calls the BB to unlock • the MTP refrigerator • Gives BB pt. information: • Name • MRN* • Gender • Charge RN or designee remains sole contact with • the BB throughout the MTP • Age • Weight • Age • Weight • Age • Weight *If available *If available *If available • Blood Bank Tech: • Prepares 1st blood order for pick-up by ICU runner • Notifies BB supervisor of MTP • Blood Bank tech who answers phone: • Unlocks MTP refrigerator in the OR or the ED • Prepares 2nd blood product supply for pick up • Notifies BB supervisor of MTP • ICU Charge Nurse: • Designates CCT as MTP runner • Sends runner to BB with pt’s • cross & match specimen • OR Charge Nurse: • Designates PST as MTP runner • Sends runner to BB with pt’s • cross & match specimen • ED Nurse Scribe: • Designates CCT as MTP runner • Sends runner to BB with pt’s • cross & match specimen Runner takes cross & match ASAP to BB & returns; stays on stand-byfor next BB run • ICU Charge Nurse assures that bedside RN: • Receives the units, • Labels units • Administers per order ED Nurse Scribe logs out blood products from MTP refrigerator, labels units, & administers, per order OR Charge Nurse logs out blood products from MTP refrigerator & delivers blood to OR Room More MTP needed? RN responsible for MTP sends runner to BB for next delivery & documents on MTP form NO YES RN responsible for MTPcalls BB to cancel MTP & documents on MTP Form END Pt. to be transported with blood to OR or ICU? YES NO RN responsible for MTP transports blood with pt. & hands off to accepting RN & documents on MTP Form END
