4. Changes to the CSD 43 Intake Form Please ensure that the CSD 43 Intake form is completely filled out and is the most recent version updated on 1/06/2012.
First thing we did was took your advice and made the form smaller, now it is 8 ˝ by 11.
Agency name is unlocked for your convience.
SDI has been added to the Income Categories.

5. Changes to the CSD 43 Intake Form All of the Weatherization information has been removed, with the exception of “Type of Dwelling”.
The “Agency Defined” categories are now unlocked to allow agencies to type in their specific defined categories.
The section for “Official Use Only” has been moved to the bottom of the form, below the applicant signature.
The Program Year has been removed from the top of the form, being that the intake form will be used for all current program years.

7. Household Composition
A household is defined as an individual or group of individuals related or unrelated, who function as one economic unit, who share residential energy, and who have an energy cost.
8. Household Composition Exceptions
Live-in Attendants - Individuals who reside with a household to provide necessary medical services and whose services are paid for in part or in full by a third party, i.e. Veterans Affairs Aid & Attendants, In Home Supportive Services. There are two qualifying factors.
1.) You have a permanent residence elsewhere.
2.) Your sole reason for being part of the household is to provide care to the recipient.
Persons living in multi-unit buildings - If a building contains more than one housing unit but has only one meter or tank that is shared by all the units, each unit may contain a separate household if each one functions as a separate economic unit.
9. Household Composition Exceptions, Continued…
A person out of the home for reasons of employment, education, hospitalization, etc. who continues to support or be supported by the unit and who intends to return to the unit will remain a member of the household.
A dependent child who is a student living away from his/her primary residence to attend an educational facility is considered to be a member of the primary residence.
10. Household Composition Ineligible Households
Subsidized households that do not pay any out of pocket energy costs.
Persons living in licensed facilities (nursing homes, assisted living, etc.).
Persons living in temporary shelters or group homes who have no energy expense or who pay a nominal fee to live there.
Persons who reside in only one room within a larger dwelling and is not considered a separate household.
Persons who have no physical address.
11. Completing the Intake form CSD 43 The purpose of the CSD 43
To assess a household’s need and to determine eligibility for LIHEAP and DOE services. The applicant is the primary source of information in establishing eligibility for LIHEAP and DOE assistance and must furnish the required verification to establish household eligibility.
12. Completing the Intake form CSD 43 The agency name and initials of the intake worker must be included on the intake form.
The intake date can be one of two dates:
The date in which the application was received by the agency
The date in which the application is considered complete
The Eligibility Cert Date is to be used for Utility Assistance and Weatherization applications. The date in which the client is eligible to receive services.
Agencies must continue to request a nine digit Social Security Number (SSN) from the applicant. The last four digits of the SSN are not acceptable.
13. Completing the Intake form CSD 43 Demographics Section Includes the applicant and all other members of the household.
The number of members in the household receiving income must be recorded.
Total gross monthly household income from all sources for all members of the household over 18 years of age.
The Utility information must be included.
The applicant’s signature is required.
14. Completing the Intake form CSD 43 The For Official Use Only section:
Program Type
Cash Assistance – HEAP, Fast Track
Weatherization – DOE, DOE ARRA, LIHEAP WX, ECIP HCS
Check the appropriate dwelling type of the household applying for services.
Total of the most current energy costs, including all sources of energy used in the household. A copy of the energy bill for the company selected for benefits must be in the client file.
15. Completing the Intake form CSD 43 The For Official Use Only section:
In accordance with the agency’s LIHEAP Priority Plan, select which agency defined category or categories the applicant falls under.
For Cash Assistance, the intake worker must include the points the applicant received based on the agency’s priority plan.
For all Weatherization programs, the agency’s internal code for each job.
16. Common Mistakes on the CSD43 Use of white-out to fix an error
Correct Procedure: Line through and initial. Write the correct information as close as possible.
Example: Income: $1200JZ $1150
Every member of the household is not being accounted for in the demographics section.
Energy burden not being calculated correctly. Total of demographics should equal or greater then Household size. Demographics would never be less then HH size.
Energy Burden = Divide total monthly energy costs from all sources by total monthly household GROSS income.Total of demographics should equal or greater then Household size. Demographics would never be less then HH size.

17. Undocumented Sources of Income Cash income from irregular sources including but not limited to, babysitting, mowing lawns, redeeming cans/bottles, paid in cash or selling personal property.
A client self certification statement CSD 43B, may be used for proof of income when the client does odd jobs from various sources and cannot obtain verification.
Acceptance of this type of verification must be fully justified and documented in the client file.
18. Undocumented Sources of Income New DOE Policy
In accordance with The Department of Energy, Weatherization Program Notice 10-18, beginning December 2011 all applicants applying for DOE/DOE ARRA services with undocumented income or zero income, only after all other avenues of documenting income eligibility are exhausted, self-certification is allowable. Evidence of the various attempts at proving eligibility must be contained in the client file, including a notarized statement signed by the applicant that they have no other proof of income.
LIHEAP applicants must also use the CSD 43B but are not required to have the form notarized.
19. Zero Income Applicant households with zero income or expenses that exceed the income, requires the intake worker to determine how the household is meeting its current living expenses.
Example: Client has $2000 worth of expenses and $500 worth of income.
Current living expenses include but are not limited to: rent/mortgage, utilities (gas, power, trash, and phone), food, insurance and car payments.
This procedure does not apply to households that submit proof of non-countable income and/or excluded income, i.e Food Stamps, Foster care.
The verification of non-countable income or excluded income must be included in the client file.
20. Zero Income
The first year an applicant claims no income, the applicant may declare their sources of support on the Survey of Income and Expenses CSD 43B, without documentation. The succeeding years documentation must accompany the CSD 43B.
Applicants may be denied services if inadequate information is provided.
For DOE applicants claiming no source of income, the CSD 43B must be notarized. Although it is not required , It is best practice that all adults in the household claiming no income also submit a completed CSD 43B but it does not need to be notarized.
Reminder
LIHEAP Applicants must also use the CSD 43B but are not required to have the form notarized.
21. Fraud
Fraud is providing false information with the knowledge that the information is incorrect or withholding information that could affect eligibility.
It is the agencies responsibility to be aware of potential fraud.
22. 2012 Fact Sheets The 2012 fact sheets have been changed to allow local service providers to input their contact information on the blank line. Example Below:
The local community services agencies are responsible for processing applications and the Department of Community Services and Development (CSD) is responsible for issuing HEAP payments. To find out how to apply for services, please call 1-800- your – agency
Free weatherization services are available to low income property owners and renters. Eligibility is based on the household’s total monthly income, which cannot exceed the income guidelines listed below. To find out if you qualify, you must contact your local service provider. To find out how to apply for services, please call 1-800-your -agency
23. 60% State Median Income
The Income Guidelines for Program year 2012 are set for 60% of the 2011 State Median Income.
Although the income guidelines are set at 60% SMI for 2012 which is the same as 2011, The California Median Income went down.
These guideline became in effect as of Jan 3rd, 2012.
Exception: For all ongoing contracts, continue to use the Income Guidelines that are in effect for that contract.
