  2. Jun. 21 – Jun. 27 Fremont June 23, 2014 Cooling zone blower belt guard fell off motor onto floor (tab broke off bracket where it attaches to blower housing) Guard fell 15' to floor. No one was in area when incident occurred. Checked rest of blower guards for similar issue - all ok.

  3. Jun. 21 – Jun. 27 Olive Branch June 24, 2014 EE was doing a 360 degree inspection, when EE noticed a conveyor panel was missing. An accident was averted due to the EE's awareness of their surroundings. Maintainer was notified and the conveyor cover was put in its proper place and secured.

  4. Jun. 21 – Jun. 27 Chatsworth June 25, 2014 Very small fire in 12oz washer AC Unit. Removed AC unit power cord from inside cabinet but fire continued. Tried to extinguish fire from outside of unit but not successful. Donned gloves and partially removed cover and used extinguisher to put out fire.

  5. Jun. 21 – Jun. 27 Chicago June 25, 2014 EE was pulling a trimmer jam and opened the safety housing up to access the jam. EE pulled jam and jogged BM to ensure good flow. EE started up BM but left trimmer guard open.

  6. Jun. 21 – Jun. 27 Longview June 27, 2014 Pin Stripper drive chain guard left off while equipment was running and employees working in area. Back End Maintainer reinstalled guard.
