Current Arrangements forCommunity Pharmacy • Independent contractors and practitioners • New contracts based on “necessary and desirable” criteria • Current contract based on supply function • NHS remuneration (mainly) derived from dispensing prescriptions (currently 94.6p per prescription item) • Pharmacies use purchasing power to buy drugs at a discount price • Cost of drugs reimbursed at Drug Tariff rates less a discount factor

  5. A Vision for Pharmacy in the New NHS “Pharmacy is an integral part of the NHS family. We want to see pharmacists strengthening their contribution to the provision of high quality, patient centred NHS services –in community pharmacies, in other primary care settings and in hospitals”

  8. Why A New Pharmacy Contract? Because…… The current contract = Disincentive to reduce dispensing Focus on dispensing fee Emphasises workload and throughput Treats all community pharmacies the same

  9. Proposals to Reform and Modernise the NHS (Pharmaceutical Services) Regulations 1992 • Balanced package of measures • New criteria for competition and choice • Four exemptions • Further reform and modernisation

  10. To introduce new criteria of competition and choice for a “necessary or desirable” pharmacy within the current regulatory framework. Qu1 – Does the application meet the minimum expected essential and, in due course, undertake to provide the enhanced levels of service provision within the proposed new contractual framework for community pharmacy. Qu2 – Does the application lead to the provision of additional or higher quality services in the relevant neighbourhood as a whole, and/or does it increase choice and competition in the relevant neighbourhood.

To exempt applications from pharmacies from the current control of entry requirements • Pharmacies in large shopping developments over 15,000 m2 • Pharmacies that intend to open for more than 100 hours a week • Applications from members of a consortium to establish one of the new one-stop primary care centres

  12. To reform and modernise the control of entry regulation provisions. The aim is to make the regulatory system more business-friendly, to provide more certainty and reliability for companies who depend upon them and to make the process less time-consuming, ensuring PCT decisions were taken quickly and with ready access to a sound appeals mechanism.
