Health History Overview • Dizziness, nervousness, stress • Pain, stiffness in neck and shoulders • For 5 years now • Severity rated as 7 out of 10
Short-Term Goal • The client will perform neck, shoulder, and upper back stretches every day for 15 minutes each time in order to relax muscles and reduce stiffness and pain in that area. We will know the goal has succeeded if the stretches reduced pain significantly by the end of May.
