  4. Preserve & Test Service Line • Preserve the service line that served the house, and make it available for inspection by the WUTC upon request and any consultant retained by the Commission. (Section 16 c.) • Conduct a metallurgical analysis of the service line that served the house to determine what caused it to leak. (Section 16 d.)

  5. Investigation Records PSE will preserve and make available to the WUTC upon request: • Any and all investigation records of the explosion and its causes. (Section 16 f.) • Any and all documents surrounding any investigation of the explosion, and/or the rectifier, and/or any of the other facilities involved. This includes any leak surveys that are conducted. (Section 16 g.)

  6. Rectifier Records Preserve and make available to the WUTC all records relating to the installation and maintenance of the rectifier, including any record containing information on the period when the rectifier failed to provide the level of cathodic protection required by Commission rules. (Section 16 e.)

  7. Communication Plan PSE must within ten days of the order (issued 9/17/04): • Alert affected customers that the problem may be broader than a single house. • Inform customers of the actions they should take if they smell natural gas. • Send periodic updates on the status of remedial actions and the results of the company’s investigation. (Section 16 a.)

  8. Leak Surveys Conduct leak surveys of all mains and service lines served by the rectifier on or before Oct 4, 2004, and every 30 days after, until the Commission orders otherwise, and provide the results to the WUTC upon request. (Section 16 h.)

  9. Repair Leaks Served by Rectifier • Repair all leaks detected and verified in any of the facilities served by the rectifier. • Apply for permits no later than the day following leak discovery. • Begin to repair leaks no later than the day following receipt of permits and locates. • Consult with WUTC when detected leak cannot be located. (Section 16 b.)

  10. Pipeline Coating Conditions Conduct a test(s) that will determine the condition of the coating of the coated steel service lines and mains in the area covered by the rectifier. (Section 16 i.)
