If a Voter Did Not Bring Acceptable Identification Give the Voter the option to complete and sign the Personal Identification Affidavit
On the Affidavit the voter swears to their identity under penalty of perjury The name must match name in the Poll Book When completed and signed direct the voter to the issuing clerk for a ballot
A Voter May Not Be Refused Their Right to Vote Unless AND Did Not Bring Acceptable Identification Declines To Complete Affidavit
