Structure of OASYS • 3 Forms: • AFL Lesson Plan Template (Only Planning and Planning + Environment): Completed by teacher and submitted to evaluator; • Observation Form: Completed by evaluator and acknowledged by teacher; • Self-Score Form: Completed by teacher and submitted to evaluator. • All forms are scheduled by evaluator; • Schedule must be ‘accepted’ by teacher.
Announced Observation: Accepting Scheduled Forms • Evaluator must schedule all 3 forms in order for you to access (2 forms if no Lesson Plan); • Click ‘Accept Schedule’ for all 3 forms and pay careful attention to notes; • If you must ‘decline schedule,’ be sure to add your own notes to document the reason for declining.
AFL Lesson Plan: Planning and Planning + Environment • After ‘accepting’ the schedule, the form will be in one of 2 places: • Scheduled: if the date for which the form was scheduled is in the future; • Awaiting Action: if the date for which the form was scheduled is today or a day in the past; • When the form is in ‘Awaiting Action,’ Click ‘Submit the AFL Lesson Plan’ to open up the form and begin completing it. • Click ‘Save’ to return to the form later; • Click ‘Save and Notify’ if the form is not fully complete but you would like for your evaluator to be able to review and/or comment; • Click ‘Submit’ when the form is fully complete and ready to be evaluated
Announced Observation: Observation Form • Evaluator will schedule the form so that you know when s/he will conduct the actual observation; • Pay careful attention to the ‘Notes’ for dates of pre-conference and post-conference; • You cannot type on the Observation Form, and the evaluator will not submit the completed form to you until after the post-conference has concluded.
Unannounced Observations • None of the forms will be scheduled in advance in an unannounced observation; • All forms will be scheduled on or shortly after the observation actually occurs; • You will need to ‘accept’ the schedules at this time: • the date of the post-conference may be included in the ‘Notes’ of the Observation Form; • The date on which your Self-Score is due may be included in the ‘Notes’ of the Self-Score form.
Submit Your Self-Score • The Self-Score will be available for you to complete on the date of your actual observation; • Click ‘Submit Self-Score’ to open the form in a new window; • You can save as a draft, but in order to ‘Submit,’ you must complete all required fields: • Date of Observation; • Title of Lesson; • Evidence for each indicator (click ‘View Evidence’ to type in your evidence) • A score for each indicator based upon your evidence; • Area of Reinforcement (Strength) • Area of Refinement.
Acknowledge the Observation • After the post-conference has concluded, you will be prompted to ‘acknowledge’ the observation; • To do this, click ‘Acknowledge Observation’ from the ‘Action Required’ area of MLP; • Acknowledge means only ‘receipt’; • It does not indicate agreement or disagreement with the evaluation. • Before you ‘Acknowledge’ be sure to include any statement that you wish to attach by clicking ‘Comment.’
Comment • If you include a comment on any form (including the observation), it will be a part of that form visible to both you and the evaluator:
Accessing Forms After Completion • After the post-conference has concluded and the evaluator has ‘finalized’ all of the forms, that piece of the evaluation will be marked ‘Complete’ on My Learning Plan; • You can still view, print or comment on any form at any point from that date forward by clicking on the form:
Moving Forward • In the future, you will be able to access this year’s evaluation forms from your ‘My Evaluations’ link on MLP; • You can also access any current forms by clicking on this year’s evaluation from this link
